白内障摘除及小梁切除术式治疗老年急性闭角型青光眼的临床比较分析

Clinical comparative analysis of cataract extraction and trabecniectomy for elderly acute angle closure glaucoma

目的：对比分析白内障摘除及小梁切除术式治疗老年急性闭角型青光眼的临床疗效及特点．方法：回顾性分析93例（98眼）老年急性闭角型青光眼患者的临床病例资料，行小梁切除术的39例（41眼）设为对照组，行白内障摘除的54例（57眼）设为观察组，治疗后随访6个月对比两组疗效．结果：观察组治疗后最佳矫正视力、眼压、前房深度等均治疗前明显改善，且均优于对照组，差异均有统计学意义（P〈0．05）．结论：白内障摘除术治疗伴白内障的老年急性闭角型青光眼疗效优于小梁切除术式，临床应用可根据适应症优先选择．

AIM： To comparatively analyze the clinical effects and features of cataract extraction and trabeculectomy for elderly acute angle closure glaucoma. METHODS： We retrospectively analyzed clinical data of 93 cases （98 eyes） elderly patients with acute angle closure glaucoma. Regarding the 39 cases （41eyes） who had received trabeculectomy as contrast group, while the 54 cases （57eyes） with cataract extraction as observation group. The efficacy of two groups were compared after treatment six months follow-up. RESULTS： After treatment, the best corrected visual acuity, ocular pressure and anterior chamber depth of observation group improved a lot, which better than contrast group. The differences were statistically significant （P 〈 0.05 ）. CONCLUSION： Cataract extraction for the treatment of elderly acute angle closure glaucoma with cataract is better than trabeculectomy. And in clinical, the cataract extraction could be considered firstly.
