8周不同处方训练对半月板术后患者膝关节本体感觉和等张功率的影响 被引量:10

Effects of Eight Weeks Different Training Prescription on Knee Joint Proprioception and Isotonic Power of Meniscectomy Patients

摘要 目的:探讨8周常规康复处方和本体感觉加强处方训练对膝关节半月板术后患者本体感觉和等张功率的影响,科学指导膝关节半月板术后患者的临床康复训练。方法:25名半月板术后患者,依照均衡原则分为常规康复训练组(13人)和本体感觉加强训练组(12人),在常规康复训练(以肌力和关节活动度训练为主)和本体感觉加强训练(以本体感觉训练为主)不同时间点使用BIODEX PRO 3 SYS测定膝关节被动60°位置觉(术前和术后4、8周)及等张功率(术前和术后8周),使用X-SENS测定运动觉(术前和术后2、4、8周)。结果:处方实施4周后本体感觉明显加强,4~8周变化趋势最明显。8周不同处方训练后,本体感觉加强组患侧被动位置觉和运动觉60°、常规康复组患侧被动60°位置觉均有显著性加强(P〈0.01),不同处方训练对患侧被动60°位置觉和运动觉60°的影响有显著性差别(P〈0.01);常规康复训练和本体感觉加强训练均对患者患侧等张功率(伸/屈)有影响(P〈0.05),但不同处方训练对患侧等张功率(伸/屈)的影响无差异。结论:8周不同处方训练均能有效改善半月板术后患侧膝关节本体感觉功能;同常规康复训练处方相比,8周本体感觉加强训练处方能显著改变患侧本体感觉功能;8周不同处方训练后患侧等张功率(伸/屈)均有显著加强,与本体感觉功能加强有关,应加强本体感觉训练在半月板术后康复中的应用。 Objective: This study focused on the effects of eight weeks conventional rehabilitation training and prop rioeeption improving training on knee joint proprioeeption and isotonic power, in order to provide a clinical rehabil itation training guide for meniscectomy patients. Methods: Twentyfive clinical meniscectomy patients were divided into two groups: conventional rehabilitation training group (n = 13 ) and proprioception improving training group ( n = 12). Passive 60° position sense and isotonic power of knee joint were measured before surgery, 4 weeks after meniscectomy, and 8 weeks after meniscectomy by BIODEX PRO 3 SYS. Motor perception of knee joint was meas ured before surgery, 2 weeks, 4 weeks, and 8 weeks after meniscectomy by XSENS. Results: Proprioception is improved after 4 weeks training, and its most obvious change trend is from 4 to 8 weeks. After 8 weeks training, passive 60° position sense and 60°motor perception of knee joint with meniscectomy are enhanced significantly in both training groups (P 〈 0.01 ), and they are different in both training groups ( P 〈 0. 01 ). Both training pro grams influence isotonic power in knee joint with meniscectomy ( P 〈 0. 05 ), but no different between two groups. Conclusion : Eight weeks different training prescription can improve knee joint proprioception of meniscectomy. The proprioception improved by proprioception is better than by conventional rehabilitation training. Eight weeks differ ent trainin can enhance the isotonic Dower (extensor/flexor) significantly, which is associated with the improvement of proprioceptive. Proprioception improving training should be paid more attention in clinical rehabilitation in menisceetomy patients.
