软骨MRI敏感序列在距跟骨间韧带治疗中的应用

Application of cartilage-sensitive MRI sequences in treatment of talocalcaneal interosseous ligament

摘要 目的探讨软骨磁共振成像(MRI)敏感序列在距跟骨间韧带治疗的应用指导效果。方法选择2009年8月—2013年1月湖北医药学院附属东风医院收治的跟骨骨折伴有距跟骨间韧带损伤患者60例,都进行MRI扫描,选择MRI三维容积式内插法屏气检查(T1-VIBE)序列、三维自旋回波序列(3D-SPACE)和三维动态增强(3D-FLASH)序列扫描足踝部,对图像质量进行评分,对距跟骨间韧带长度和宽度进行最优序列的测量。结果 3D-SPACE和3D-FLASH序列的图像评分高于T1-VIBE序列(P<0.05),3D-SPACE和3D-FLASH序列的图像评分组间对比差异无统计学意义(P>0.05)。在同性别者,中距跟骨间韧带长度和宽度左侧与右侧无明显差异(均P>0.05)。结论软骨MRI 3D-SPACE和3D-FLASH敏感序列有较好的空间分辨率,对距跟骨间韧带长度和宽度有很好的测量指导,可以作为距跟骨间韧带病变的首选扫描序列。 [ Objective] To investigate the guidance effect of cartilage-sensitive MRI sequences in treatment of talocalcaneal in- terosseous ligament. [ Methods]60 cases of calcaneal fractures with interosseous talocalcaneal ligament injury, which were treated in the Dongfeng Hospital Affiliated to Hubei Medical College from August 2009 to January 2013, were given the MRI scans. T1- VIBE sequence, 3D-SPACE sequence and 3D-FLASH sequence were applied to scan the ankle. The image quality scores were evaluated, and the optimal test sequence was used to measure the length and width of interosseous talocalcaneal ligament. [ Results] The image quality scores of 3D-SPACE sequence and 3D-FLASH sequence were higher than those of T1-VIBE sequence (P 〈0.05). There was no significant difference in image quality scores between 3D-SPACE sequence and 3D-FLASH sequence ( P 〉 0.05 }. In the same sex, the differences in the length and width of interosseous talocalcaneal ligament between the left and right sides were not significant (all P 〉0.05). [ ConelusioB]3D-SPACE sequence and 3D-FLASH sequence in cartilage-sensitive MRI have better spatial resolution, and show the good guidance effect on measurement for the length and width of interosseous talocalcaneal ligament, which can be used as the preferred scan sequence for interosseous talocalcaneal ligament lesions.
