括约肌间瘘管结扎术与切开挂线术治疗复杂性肛瘘的疗效比较

The Clinical Comparison of Intersphincteric Fistula Ligation and the Incision Line Operation in Treatment of Complex anal Fistula

目的:比较括约肌间瘘管结扎术与切开挂线术治疗复杂性肛瘘的临床疗效,探讨复杂性肛瘘的最佳手术方式。方法:选择同期复杂性肛瘘患者80例,根据患者自愿的原则,随机均分为对照组(n=40例)和观察组(n=40例),对照组采取切开挂线术治疗,观察组采取括约肌间瘘管结扎术治疗,比较两组患者治疗后临床疗效及术后状况。结果:治疗后两组患者临床满意例数比较,差异具有统计学意义(P<0.05);平均住院时间、平均创口愈合时间、术后明显疼痛、肛门功能障碍、局部出血感染和术后6个月复发例数比较,差异均具有统计学意义(P<0.05),观察组优于对照组。结论:手术治疗复杂性肛瘘时,应首选括约肌间瘘管结扎术治疗,可提高临床疗效,促进创口愈合,减少术后并发症。

Objective： To compare the clinical effect of intersphincteric fistula ligation and the incision line operation in treatment of complex anal fistula and research the optimum operation mode. Methods： Collect 80 patients with complex anal fistula, according to the principle of voluntary, they were divided into the control group and the observation group, compared with that clinical effect after treatment and the postoperative condition. Result： There has a significant statistical difference （P〈0.05） in the clinical satisfaction cases （P〈0.05）; there has a significant statistical difference （P〈0.05） in the average hospital stay, average wound healing time, postoperative pain, anal dysfunction,local infection and recurrence after 6 months, the observation group was better than the control group. Conclusion： It should choose the intersphincteric fistula ligation when operating the complex anal fistula,it could improve the clinical effect, promote the wound healing, reduce the postoperative complications.
