HIV/AIDS合并慢性病毒性肝炎胆囊声像图特征与CD4^+T淋巴细胞数相关性的研究

Research for Relevance of Gallbladder Ultrasonic Characteristics of HIV /AIDS Combined Chronic Viral Hepatitis and CD4^+T Lymphocyte Count

目的探讨HIV/AIDS合并慢性病毒性肝炎在CD4+T淋巴细胞计数不同免疫时期胆囊的超声改变。方法对照组50例为单纯性慢性病毒性肝炎,实验组50例为HIV/AIDS合并慢性病毒性肝炎,实验组按CD4+T淋巴细胞计数的不同免疫时期进行超声观察。两组肝功能均为正常或轻度异常。超声主要观察胆囊大小,胆囊壁厚度、胆囊腔内回声。将观察的结果与临床化验进行对照分析。结果超声观察胆囊在不同CD4+T淋巴细胞计数的免疫时期的异常改变,主要表现在胆囊壁毛糙、胆囊壁稍厚、增厚,胆囊腔内回声欠清晰,细弱光点回声;实验组与对照组之间胆囊各观察指标比较,CD4+T细胞数在300/mm3以上时差异没有统计学意义,CD4+T细胞数在200~100/mm3时两项差异有统计学意义,CD4+T细胞数100/mm3以下时差异有统计学意义。结论 HIV/AIDS合并慢性病毒性肝炎的胆囊异常超声改变随着CD4+T细胞计数下降更为明显,综合考虑胆囊超声各项指标的变化有助于临床评估HIV/AIDS合并慢性病毒性肝炎的病情发展。

Objective To explore gallbladder ultrasonic change of HIV/AIDS Combined chronic viral hepatitis in count different immune period of CD4^＋ T lymphocyte. Methods The control group of 50 cases of simple chronic viral hepatitis;the experimental group of 50 cases of HIV/AIDS combined chronic viral hepatitis, according to the different immune period of CD4^＋ T lymphocyte count to observate gallbladder ultrasonic change. Two groups’ liver function is normal or slightly abnormal. The ultrasonic size 、wall thickness and cavity echo of gallbladder were observed. The results and clinical test were done comparative analysis. Results Gallbladder ultrasound at different immune period of CD4^＋T lymphocyte count has abnormal change, mainly displays in the gallbladder wall is coarse or thick, and gall bladder cavity echo owes clear or thin light echo;Each gallbladder observation index’ s comparison between experimental group and control group, the comparison of CD4^＋T cells count above 300 cells/mm^3 is not statistically significant, the comparison of CD4^＋T cells count in 200-100 cells/mm^3 has two different items being statistically significant, the comparison of CD4^＋T cells count below 100 cells/mm^3 has significant difference. Conclusion The abnormal ultrasonic changes of gallbladder in HIV/AIDS combined chronic viral hepatitis are more obvious with CD4^＋ T cells count down. To comprehensively consider the change of gallbladder ultrasonic indicators would help to clinical evaluation of disease progression for HIV/AIDS combined chronic viral hepatitis.
