电针对冠心病下壁及前侧壁心电图ST-T影响的即时效应观察 被引量:15

Realtime Effect of Electroacupuncture on Inferior and Anterolateral ST-T Segments in Coronary Heart Disease

摘要 目的从经穴与脏腑相关角度,观察电针灵台、神道穴对冠心病心电图下壁和前侧壁心电图ST-T影响的即时效应差异,为临床优选腧穴提供客观依据。方法根据针刺治疗前常规心电图ST-T变化,将60例冠心病心绞痛患者分为导联组1(Ⅱ、Ⅲ、avF导联组)和导联组2(V4、V5、V6导联组),每组30例,分别观察两组针刺前和针刺后10 min心电图Ⅱ、Ⅲ、avF导联和V4、V5、V6导联ST-T的即时效应变化。结果两组针刺前后ST段比较差异均有统计学意义(P<0.05);治疗后组间ST段比较差异无统计学意义(P>0.05),两组效果相当。两组治疗前后T波比较差异均具有统计学意义(P<0.05);治疗后组间T波比较差异均具有统计学意义(P<0.01),导联组2效果优于导联组1。结论电针灵台、神道穴对冠心病心绞痛下壁和前侧壁心肌缺血状况均有改善作用,对前侧壁心肌缺血的改善效果优于下壁心肌缺血。 Objective Based on the relationship between acupoints and Zang-fu organs,to observe the difference in the realtime effects of the inferior and anterolateral ST-T segments in coronary heart disease (CHD) before and after electroacupuncture at Lingtai (GV10) and Shendao (GV11),for providing objective evidence to the selection of acupoints.Method Sixty CHD patients were divided into lead-group 1 (Ⅱ,Ⅲ,and avF leads) and lead-group 2 (V4,V5,and V6 leads),according to the pre-intervention ST-T results,30 subjects in each group.The realtime changes of ST-T segments were observed before intervention and at 10 min after intervention in the two groups.Result The ST segment changed significantly after electroacupuncture in both groups (P<0.05);there was no significant difference between the two groups in comparing the ST segment (P>0.05).The T wave changed significantly after intervention in both groups (P<0.05); there was a significant difference between the two groups in comparing T wave (P<0.01),indicating that lead-group 2 had a better efficacy than lead-group 1.Conclusion Electroacupuncture at Lingtai and Shendao can improve the inferior and anterolateral ischemia in CHD,and it produces better efficacy for anterolateral ischemia than inferior ischemia.
