胸椎黄韧带骨化症的临床特点及外科治疗

Study on clinical characteristics and surgical treatment of ossification of the ligamentum flavum in the thoracic spine

目的：探讨胸椎黄韧带骨化症的诊断及外科治疗方法。方法：回顾性分析手术治疗的胸椎黄韧带骨化症44例患者的临床资料,手术均采用椎管后壁切除术。结果：随访6～36个月,平均18个月。JOA评分术前平均4.3分,提高为术后平均9.5分,功能恢复率为77.3%。44例患者疗效优27例、良11例、可6例,优良率达86.5%。结论：临床表现结合影像学检查是诊断胸椎黄韧带骨化症的重要手段,采用＂层薄揭盖法＂进行椎管后壁切除术较为安全、可靠,手术疗效满意。

Objective：Study the diagnosis and surgical treatment of ossification of the ligamentum flavum（OLF）in the thoracic spine.Methods：Clinical data of 44patients（29male and 15female,age 34～67years）who underwent removal of posterior wall of spinal canal were analyzed retrospectively.Results：The follow-up time was 6～36months,averaged 18months.JOA scores were improved from 4.3to 9.5,and the functional recovery rate was 77.3%.The therapeutic efficacy was excellent in 27cases,better in 11and moderate in 6,and the excellent and good rate was 86.5%.Conclusion：Clinical manifestations combined with imaging examination are important in the diagnosis of OLF in the thoracic spine,and in the surgical treatment＂laminar shelling decompression＂is a safe method to remove the posterior wall of spinal canal.
