鱼金注射液对肺源性心脏病急性发作期的治疗研究

Study on Yujin Injection in treatment of pulmonary heart disease with acute exacerbation

目的观察鱼金注射液治疗慢性肺源性心脏病急性发作期的临床疗效。方法共计108例肺源性心脏病急性发作期患者纳入研究,并随机分为两组:对照组59例,鱼金注射液组49例。所有患者均给予抗感染、止咳、化痰、平喘、强心利尿等常规治疗。鱼金注射液组在上述治疗的基础上加用给予鱼金注射液治疗。比较两组患者症状体征及肺功能(FEV1/FVC)、脑钠肽(BNP)和血清炎症因子(CRP、IL-6、TNF-α)水平。结果对照组总有效率为84.7%,鱼金注射液组总有效率为85.7%(P>0.05)。相比治疗前,两组患者的肺功能均有好转(40.8±10.21 vs 56.8±11.52,41.1±10.47 vs60.7±12.33,P<0.01),血浆中BNP水平明显下降(614.7±92.54 vs 234.4±101.75,587.6±108.26 vs 227.5±97.64,P<0.01)。治疗后组间比较,鱼金注射液组显现出更好的肺功能改善作用(P<0.05),而对BNP影响不大(P>0.05)。鱼金注射液组CRP、IL-6、TNF-α水平显著低于对照组(P<0.05)。结论鱼金注射液可以明显改善患者的临床症状和体征,改善肺功能而非心功能,降低体内的炎症水平,对于治疗慢性肺心病急性发作有较好疗效。

[Objective]To observe the clinical efficacy of Yujin Injection in treatment of chronic pulmonary heart disease（CPHD）with acute exacerbation.[Methods]108 cases of CPHD with acute exacerbation were enrolled in this study and were randomly divided into the control group（59 cases） and the Yujin Injection group（49 cases）.All patients received the conventional treatment,including the anti-inflammatory,antitussive,expectorant,antiasthmatic and cardiac diuretic.The Yujin Injection group was given the Yujin Injection on the basis of the conventional treatment.The signs,symptoms,lung function（FEV1 /FVC）,as well as the levels of brain natriuretic peptide（BNP） and serum inflammatory factors（CRP,IL-6 and TNF-α） were compared between the two groups.[Results]The total effective rate of the control groupand the Yujin Injection group was 84.7% and 85.7% respectively（P 〉0.05）.Compared with before treatment,the lung function after treatment in two groups both improved（40.8 ± 10.21 vs56.8 ± 11.52,41.1 ± 10.47 vs 60.7 ± 12.33,P 〈 001）,while the plasma BNP level significantly reduced（614.7 ± 92.54 vs234.4 ± 101.75,587.6 ± 108.26 vs 227.5 ± 97.64,P 〈 0.01）.After treatment,the Yujin Injection group showed better improvement of pulmonary function than the control group（P 〈0.05）,but had little effect on BNP level（P 〉0.05）.The levels of CRP,IL-6 and TNF-α in the Yujin Injection group were significantly lower than those in the control group（P 〉0.05）.[Conclusion]Yujin Injection could significantly alleviate the clinical symptoms and signs of patients,improve the lung function rather than the cardiac function,and reduce the inflammatory factors levels.It has a good effect in the treatment of chronic pulmonary heart disease with acute exacerbation.
