过敏性紫癜患儿急性期凝血状态与肾脏功能损伤的相关性

Association of the function of coagulation with impairment of renal function in patients with acute phase Henoch-Schonlein purpura

目的探讨过敏性紫癜患儿急性期凝血状态与肾脏损伤的关系。方法选择在我院进行治疗的单纯过敏性紫癜患儿20例为单纯组,过敏性紫癜肾炎患儿20例为肾炎组,另选择20例健康儿童为对照组,检测急性发病期患者凝血相关指标以及肾功能相关指标,分析凝血功能与肾脏损伤之间的关系。结果单纯组和肾炎组血P-选择素、尿P-选择素、血TM、血浆D-D、尿D-D显著高于对照组(P<0.01)。而肾炎组尿P-选择素、血TM、尿D-D显著高于单纯组(P<0.01)。血浆TM、尿P-选择素、尿D-D与Ccr呈负相关(r=-0.487,-0.484,-0.412,P<0.01);尿P-选择素、尿D-D与尿α1-MG呈正相关(r=0.358,0.332,P<0.01)。结论过敏性紫癜患儿在急性期存在全身血液高凝状态,而肾小球内血凝异常与肾脏的损伤具有显著的相关性。

Objective To discuss association of the function of coagulation with impairment of renal function in patients with acute phase Henoch-Schonlein purpura. Methods Selected 20 cases with pure HSP as pure group, and 20 cases with HSPN as HSPN group. Twenty healthy children were selected as control group. Coagulation index and renal function index were detected. Results Levels of serum P-seleetin, urine P-seleetin, serum TM, serum D-D, urine D-D of pure group and HSPN were higher than control group（P〈0.01 ）. Levels of urine P-selectin, serum TM, urine D-D were higher than pure group（P〈0.01）, serum TM, urine P-selectin, urine D-D showed negative with Ccr（r=-0.487,-0.484, -0.412, P〈0.01 ） ; Urine P-selectin, urine D-D showed positive with α1 -MG （r=0.358,0.332, P〈0.01 ）. Conclusion Patients with acute phase Henoch-Schonlein purpura show hypercoagulability. And disorders of hemostasis of glomerulus shows association with impairment of renal function,
