住院部活血化瘀类中药注射剂不合理应用情况的调查及探析

Investigation and analysis of blood circulation class traditional Chinese medicine injections unreasonable application in inpatient department

目的：调查本院住院部活血化瘀类中药注射剂的不合理应用情况。方法从2013年1~12月本院住院部8个科室应用活血化瘀类中药注射剂的病历中随机抽取500份，对用药适应证、溶媒选择、剂量、疗程、配伍禁忌及联合用药方面不合理应用情况进行调查，分析原因并提出改进措施。结果本调查中共涉及应用活血化瘀类中药注射剂21个品种，其中以丹红注射液应用人次最多（32.8%），其次是注射用血栓通（30.6%），应用率明显高于其他种类（P＜0.05）。不合理用药比例占11.8%，表现为适应证不符、溶媒选择不合理、超剂量、超疗程及联合用药不合理。结论本院住院部活血化瘀类中药注射剂应用广泛，但存在适应证、溶媒选择、剂量、疗程、联合用药方面不合理情况，应针对不合理用药原因采取针对性的改进措施，以提高用药效果，降低不良反应，确保患者用药安全。

Objective To investigate the unreasonable injections of blood circulations class Chinese medicine in inpa-tient department of our hospital. Methods 500 medical records were randomly selected from January to December 2013 in eight sections of our hospital inpatient using blood circulation category of traditional Chinese medicine injections.In-vestigations and analysis were also made on drug indications,solvent selection,dose,duration,incompatibility and combi-nation therapy.At last,through analyzing the causes,some improvements were prompted. Results The survey involved the use of 21 classed CCP blood circulation category of traditional Chinese medicine injections of which a maximum atten-dance of Danhong injection （32.8%）,followed by injection Xueshuantong （30.6%）,the utilization rate was significantly higher than other types （P〈0.05）.The proportion of irrational drug use accounted for 11.8% which showed indications did not match,solvent choice was unreasonable,over dosage and combination treatments were unreasonable. Conclusion Blood circulation class of traditional Chinese medicine injections are widely used in the inpatient department.However, there are unreasonable applications in indications,solvent selection,dosage,treatment and combination therapy.Improved solutions should be taken to improve drug effects,reduce adverse reactions and ensure patient medication safety.
