甲泼尼松龙在重症哮喘急性发作院前急救中的应用价值分析

Application value analysis of meprednisone in pre-hospital care for acute attack with severe asthma

目的：评价甲泼尼松龙在重症哮喘急性发作院前抢救中的应用价值。方法选取2012年7月~2013年9月本院接诊的重症哮喘急性发作患者77例，在到达救治现场后迅速给予甲泼尼松龙注射，观察和比较患者救治前、上救护车后和到达医院时的呼吸频率、心率和临床疗效评分。结果患者救治前呼吸频率为（31.9±7.1）/min、心率为（137.1±9.8）/min、临床症状评分为（9.2±1.1）分，上救护车后呼吸频率为（26.7±5.2）/min、心率为（118.8±8.4）/min、临床症状评分为（7.5±0.9）分，到达医院时呼吸频率为（22.8±4.4）/min、心率为（104.8±5.6）/min、临床症状评分为（6.3±1.0）分；患者上救护车后和到达医院时的呼吸频率、心率、临床症状评分与救治前比较，差异有统计学意义（P＜0.05）。结论甲泼尼松龙在重症哮喘急性发作院前抢救中的应用价值高，能有效提高患者的呼吸效率，解除缺氧症状，值得临床推广应用。

Objective To evaluate the application value of meprednisone in pre-hospital care for acute attack with se-vere asthma. Methods Seventy-seven cases of patients with acute attack of severe asthma in our hospital from July 2012 to September 2013 were selected.They were promptly injected with meprednisone by ambulance personnel who re-sponded to the scene.The respiratory rate（RR）,heart rate（HR）and clinical efficacy score before rescue,in ambulance and on arriving at hospital was observed and compared respectively. Results Before rescue,the RR was （31.9±7.1）times per minute,HR was（137.1±9.8）times per minute and the score of clinical symptom was（9.2±1.1）points.The RR,HR,and score of clinical symptom in ambulance was（26.7±5.2）times per minute,（118.8±8.4）times per minute and（7.5±0.9）points re-spectively.When arriving at hospital,RR,HR,and score of clinical symptom was（22.8±4.4）times per minutes,（104.8±5.6）times per minute,and （6.3±1.0）points respectively.The value of RR,HR and clinical symptom score before rescue compared with that in ambulance or arriving at hospital respectively,with statistical difference （P〈0.05）. Conclusion With a high application value of meprednisone in pre-hospital care for acute attack with severe asthma,it can effectively improve patient’s efficiency of respiratory and relieve anoxic symptom,which is worthy of expansion and application in clinic.
