子宫内膜异位症复发影响因素及其护理对策

Endometriosis recurrence factors and nursing strategies

目的探讨腹腔镜保守手术治疗子宫内膜异位症(EMS)术后复发的相关因素及护理对策。方法回顾性分析深圳市福田区妇幼保健院2010年6月-2011年6月收治的84例EMS患者,术后对其随访2年,采用单因素及Logistic多因素分析影响患者术后复发的原因,并对其实施相应的护理。结果 84例患者术后复发26例(30.95%),经单因素分析可知,影响内膜异位症保守治疗效果的相关因素包括年病程、年龄、盆腔病变情况、术后r-AFS分期、术后孕激素应用、后穹窿触痛结节,而与孕次及EMS类型无关系。经Logistic分析可知,病程、盆腔病变情况、术后r-AFS分期是影响EMS患者腹腔手术术后复发的独立危险因素。结论子宫内膜异位症复发受多种因素影响,通过术前对患者行心理护理、术中严密观察患者病情、术后对患者进行健康指导可有效降低EMS患者术后复发率。

[ Objective ] To evaluate laparoscopic conservative surgery for endometriosis （EMS） related factors of recurrence and nursing. [ Methods ] A retrospective analysis of our hospital from June 2010 to June 2021 were treated 84 cases of EMS patients for the study, its follow-up two years after using univariate and multivariate analysislLogistic affect postoperative recurrence reasons, and its implementation of appropriate care. [ Results } 84 patients recurrence in 26 cases （30.95%）, by univariate analysis, factors affecting the conservative treatment of endometriosis include years duration, age, pelvic lesions , after r-AFS stage, progesterone after application, the posterior fomix tenderness nodules, but there was no relationship between pregnancies and EMS types. After Logistic analysis, duration, pelvic lesions , after r-AFS stage were independent risk factors for postoperative abdominal surgery patients with relapsed EMS effects. [ Conclusion ] The recurrence of endometriosis affected by many factors, preoperative psychological care of patients, close observation of the patient＇s condition intraoperatIve and postoperative patient health guidance can effectively reduce the recurrence rate in patients with EMS.
