穿透性主动脉硬化性溃疡临床分析

Common and Rare Clinical Manifestations of Penetrating Atherosclerotic Ulcer

目的探讨穿透性主动脉硬化性溃疡(PAU)常见及少见临床表现。方法根据PAU发生的部位、形态和数量,分析13例PAU患者相应的临床表现。结果 13例PAU中,常见临床症状是:胸背部或上腹部疼痛9例,胸闷、气短6例,头晕、黑视2例,左上肢无脉2例,偏瘫1例,下肢无力2例。少见临床症状是:咯血1例,进食哽咽感1例。常见临床体征及检查异常是:主动脉增宽6例,胸腔积液5例,心包积液2例,透视下左侧支气管上翘1例,主动脉瓣杂音1例。结论 PAU的临床表现各异,高危人群应警惕PAU存在的可能。

Objective To investigate the common and rare clinical manifestations of penetrating atherosclerotic ulcer（PAU）. Methods Clinical manifestations of 13 patients with PAU were studied retrospectively,combined with PAU area, shape, number, etc. Results Common clinical symptoms were： pain on the back of the chest or abdominal in 9 cases, chest tightness, shortness of breath in 6 cases, dizziness, blackout in 2 cases, left upper limb asphygmia in 2 cases, hemiplegia in 1 case, lower limb weakness in 2 cases. Rare clinical symptoms were： haemoptysis in lcase,dysphagia in 1 case. Common clinical signs and checked exception were： Aortic broadening in 6 cases, pleural effusion in 5 cases and pericardial effusion in 2 cases, the left bronchus warped on perspective in l case, aortic valve noise in t case. Conclusion Penetrating atherosclerotic ulcer clinical presentation were differently. People at high risk should be alert to the possibility of PAU.
