摘要
目的分析白细胞介素17(IL-17)在早期未治疗类风湿关节炎(RA)患者血清和滑膜液中的表达水平及临床意义。方法收集2012年6月至2013年6月在四川省人民医院门诊或住院部就诊的早期未经治疗的RA患者的血清和滑膜液标本,以同期就诊骨关节炎(OA)患者为对照,共纳入30例未经治疗的RA患者和30例年龄性别匹配的OA患者作为对照。应用酶联免疫吸附法(ELISA)检测RA患者各标本中IL-17表达水平,同时分析IL-17表达水平与超敏C反应蛋白(hs-CRP)、类风湿因子(RF)以及疾病活动度DAS28评分的关系。结果在本组病例中,RA患者不仅血清中的IL-17水平明显高于OA患者[(289.5±103.2)ng/L∶(86.4±30.3)ng/L,P<0.05],而且滑膜液中的IL-17水平亦显著高于OA患者[(336.4±105.3)pg/ml∶(95.7±22.5)pg/ml,P<0.05]。RA患者血清中hs-CRP和RF水平分别为(44.6±12.3)mg/L和(215.7±56.2)U/L,DAS28评分为(5.6±1.5)分;直线回归分析显示RA患者血清和滑膜液中IL-17水平与hs-CRP、RF及DAS28评分均呈正相关(P<0.05)。结论 IL-17水平在早期未治疗RA患者血清及滑膜液中明显增高,IL-17可作为RA患者病情活动的监测指标之一。
ObjectiveTo study the expression and clinical signiifcance of interleukin17 (IL-17) in serum and synovial lfuid samples of patients with rheumatoid arthritis (RA).MethodsThe serum and synovial lfuid samples of untreated patients with rheumatoid arthritis from our hospital during June 2012 and June 2013 were collected, and patients with osteoarthritis (OA) were included as controls. A total of 30 eligible RA patients and 30 age and sex-matched patients with osteoarthritis were included. And the expression of IL-17 in serum and synovial lfuid samples were examined by enzyme-linked immunosorbent assay, and the correlation of IL-17 levels with high-sensitivity C-reactive protein (hs-CRP), rheumatoid factor (RF) and disease activity score DAS28 were also analyzed.ResultsIn this cohort, serum IL-17 levels were signiifcantly higher in RA patients than OA patients [(289.5±103.2) ng/L∶(86.4±30.3) pg/ml,P〈0.05], and the levels of IL-17 in synovial lfuids were also signiifcantly higher in RA patients than OA patients [(336.4±105.3) pg/ml∶(95.7±22.5) pg/ml, P〈0.05]. In RA patients, the levels of serum hs-CRP and RF were (44.6±12.3) mg/L and (215.7±56.2) U/L, respectively; and DAS28 score was (5.6±1.5). And linear regression analysis showed that IL-17 levels in serum and synovial lfuid were both positively correlated with serum hs-CRP, RF and DAS28 score in RA patients (P〈0.05).ConclusionIL-17 levels are signiifcantly higher in serum and synovial lfuid of RA patients, and IL-17 would be used as an indicator for monitoring the disease activity of RA.
