全髋关节置换术与骨折内固定术对于股骨颈骨折患者功能恢复情况的影响分析

Functional recovery of total hip arthroplasty and internal fixation on transcervical fracture patients

目的研究全髋关节置换术(THA)与骨折内固定术(IF)对于股骨颈骨折(TF)患者功能恢复情况的影响分析。方法选择本院2011年7月至2013年7月收治的128例TF患者进行观察,随机分为两组。观察组给予THA治疗;对照组给予IF治疗。对比分析两组患者手术情况、功能恢复情况以及疗效。结果观察组患者术后下床活动时间为(20.1±5.2)天,较对照组患者[(90.3±15.1)天]明显缩短,差异具有显著性(P<0.05)。观察组患者日常生活能力量表(ADL)评分为(55.6±15.2)分,Fugl-Meyer运动功能评定量表(FMA)评分为(76.2±23.6)分,功能独立性评价量表(FIM)评分为(58.6±11.6)分;较对照组患者的(41.1±14.2)分,(47.3±25.1)分和(40.2±11.4)分明显提高,差异具有显著性(P均<0.05)。观察组优良率为78.13%(50/64),较对照组[46.88%(30/64)]明显提高,差异具有显著性(P<0.05)。结论 THA治疗中老年TF疗效显著,能够缩短患者术后下床功能锻炼时间,促进患者机体功能恢复,值得临床推广。

ObjectiveTo study the functional recovery of total hip arthroplasty （THA） and internal ifxation （IF） on transcervical fracture （TF） patients.MethodThe total of 128 cases TF patients in our hospital from July 2011 to July 2013 were randomly divided into two groups. The observation group was treated with THA; the control group was treated with IF, compared the operation, functional recovery, and curative effect of two groups. ResultsThe observation group＇s activities time [（20.1±5.2） d] was lower than control group＇s [（90.3±15.1） d]. The observation group＇s ADL [（55.6±15.2） points], FMA [（76.2± 23.6） points], FIM [（58.6±11.6） points] were higher than control group＇s [（41.1±14.2） points, （47.3±25.1） points, （40.2±11.4） points]. The observation group＇s excellent and good rate [78.13% （50/64）] was higher than control group’s [46.88% （30/64）]. The differences were statistically signiifcant （P〈0.05）.ConclusionTHA desirable a good effect on TF patients, can effectively shorten the ambulation time, promote the functional recovery. It is worthy of recommendation.
