术前手法闭合复位在经皮椎体后凸成形术中的应用

Preoperative Gimmick Closed Reduction in the Application of the Protruding after Kyphoplasty

目的:探讨术前麻醉状态下的手法闭合复位在通过经皮椎体后凸成形术治疗老年人椎体压缩性骨折过程中的意义。方法:在2010年1月至2011年12月期间在我科接受PKP手术治疗101例老年单纯性骨质疏松性压缩性骨折患者中,选出46例压缩程度在2度以上的单节段脊柱胸腰段单纯性新鲜压缩性骨折患者,在治疗过程中按是否接受术前手法闭合复位进行随机分组,并从术后椎体高度的恢复情况,行分组对比性研究。结果:其中术前经手法闭合复位组,椎体高度压缩率的恢复和椎体后凸角度恢复率高于对照组。结论:术前麻醉状态下的手法闭合复位在经皮椎体成形术的治疗中对于椎体高度的恢复,及术后椎体双凹形的生理结构的恢复有积极的作用,对预防术后邻近椎体的骨折有无明显统计学差异尚需进一步探讨[1]。

Objective： To study the preoperative anesthesia technique under the closed reduction through protruding after kyphoplasty significance in the process of treating vertebral compression fractures in the elderly. Methods：Between January 2010 and December 2010 in our department during PKP holds the surgery treatment of 101 cases of elderly patients with simple osteoporotic compression fractures, selected 46 cases of compression degree above 2 degrees of single segmental spinal column chest waist section of pure fresh compression fractures patients, according to whether to accept preoperative technique in the process of treatment of closed reduction for random grouping, and the postoperative recovery of vertebral body height, line group comparative research. Results：The preoperative after closed reduction group, recovery and vertebral body height of vertebral body compression rate of the protruding after recovery rate is higher than the control group. Conclusion：Preoperative anesthesia technique under the closed reduction in the treatment of kyphoplasty for restore vertebral body height, and postoperative recovery of physiological structure of the vertebral body double concave has a positive effect, for prevention of postoperative adjacent vertebral fractures with or without significantly statistical difference remains to be further discussed.
