足背游离皮瓣在颌面部的临床应用

The clinical application of free dorsum pedis flap to repair skin defect of maxillofacial region

目的：总结采用足背游离皮瓣移植修复颌面部皮肤软组织缺损的临床疗效。方法2008年8月-2010年6月，对8例颌面部肿物切除术后及外伤致皮肤软组织缺损患者，采用带腓浅神经的足背游离皮瓣修复。结果8例皮瓣中，2例发生血管危象，其中1例经拆除部分缝线，应用抗凝解痉药物后缓解，1例经手术探查重新吻合血管后解除；其余6例皮瓣顺利成活。术后随访12～36个月，8例皮瓣形态、质地、色泽接近面部正常皮肤，感觉恢复满意。供足负重及行走功能无影响。结论采用足背游离皮瓣移植修复颌面部皮肤软组织缺损，其外形、质地及感觉恢复良好，疗效满意，是一种理想的术式，值得推广。

Objective To summarize the clinical effect by using free dorsalis pedis flap to repair skin and soft tissue defect of maxillofacial region. Methods From August 2008 to June 2010, 8 patients with skin and soft tissue defect of maxillofacial tumor resection and trauma, were repaired by dorsal pedis skin flap with the superficial peroneal nerve. Results In 8 cases of skin flap, vascular crisis occurred in 2 cases, among 1 cases the removal of part of the suture and application of anticoagulant drugs to alleviate, the other 1 patients with vascular anastomosis after the reoperation exploration. The remaining 6 cases survived successfully. Patients were followed up for 12 to 36 months, the shape, texture and color of flaps was close to the normal facial skin, sensory recovery was satisfaction. No effect on the full weight-bearing and walking function. Conclusion It can get higher successful rate, good appearance and quality that using the free dorsum pedis flap to repair skin defects of maxillofacial region, and it＇s suitable for clinical application.
