摘要
目的:通过对3例因聚氨酯类遇水膨胀止水胶所致手指高压注射伤(high-pressure injection injuries, HPII)的治疗,旨在探讨一种合理的治疗方式。方法2011年10月-2013年9月对3例HPII患者均行早期清创,并敞开伤口,2~3 d后再行2~3次的彻底清创后,闭合创面。术后观察患指的血供和愈合时间,并根据TAM评分标准进行评定。结果本组3例患指的术后血运良好,创面平均愈合时间为22 d,TAM评分:优1例,良1例,差1例。结论采用早期、多次、逐步、彻底的清创,并延期闭合伤口的处置方式可获得良好的预后,是此类损伤的有效治疗方法之一。
Objective To report the treatment of high-pressure injection injury for finger caused by water-swelling water-proofing gel of polyurethane in 3 cases from October 2011 to September 2013 and to discuss an effective treatment. Methods All wounded fingers were performed the debridement in the early stages, wound was sutured after debridement in total 2 or 3 times. The blood supply and healing time of fingers were observed. Finger activity was evaluated according to the TAM score. Results 3 wounds had good blood supply and were healed satisfactorily in 22 days. Excellent in 1 case, good in 1 case and poor in 1 case according to the TAM score. Conclusion A favorite result can be obtained through the debridement in the early stage and repeated debridement for the patient of high-pressure injection injury caused by water-swelling water-proofing gel. It is one of effective treatments for such injury.
