卵巢癌根治术前后患者血清人附睾上皮分泌蛋白4、糖类抗原125的动态变化及意义 被引量:3

Dynamic changes and significance of serum human epididymis protein 4 and carbohydrate antigen 125 before and after radical surgery for ovarian cancer patients

摘要 目的观察卵巢癌根治术前后患者血清人附睾上皮分泌蛋白4(HE4)、糖类抗原125(CA125)的动态变化,探讨其临床意义。方法 (1)以43例卵巢癌根治术患者为观察组,241例卵巢良性疾病手术患者为对照组,检测两组患者手术前血清HE4、CA125水平,并进行组间比较;(2)观察组于术后1个月、3个月、6个月、12个月、18个月、24个月随访检测血清HE4、CA125,将各时点检测结果与术前检测结果以及前一时点的检测结果进行比较。结果 (1)观察组术前血清HE4、CA125水平明显高于对照组,差异有高度统计学意义(P<0.01);(2)观察组术后1个月、3个月、6个月、12个月血清HE4、CA125水平逐渐下降,此4个时点的检测结果与术前比较均有高度统计学意义(P均<0.01),且各时点检测结果与前一时点检测结果比较亦有高度统计学意义(P均<0.01);术后18个月、24个月两个时点血清HE4、CA125水平有所回升,但与前一时点的检测结果比较,差异无统计学意义(P均>0.05)。(3)观察组手术12个月后有9例出现复发,复发者术后18个月、24个月两个时点血清HE4、CA125水平再度升高,与未复发者(n=34)比较有高度统计学意义(P均<0.01)。结论血清HE4、CA125联合检测可以作为卵巢癌早期筛查、手术疗效评价和复发监测的重要手段。 Objective To observate dynamic changes of serum human epididymis protein 4 and carbohydrate antigen 125 before and after radical surgery for ovarian cancer patients, and explore its clinical significance. Methods (1)43 cases patients of ovarian cancer radical surgery were included in the observation group,241 cases patients of ovarian surgery for benign disease were included in the control group.To detect the levels of serum HE4 and CA125 of two groups patients before surgery,and to compare the result of the detection between the two groups. (2)To follow-up and examine the levels of serum HE4 and CA125 of the observation group at 1 month after surgery,3 months after surgery,6 months after surgery,12 months after surgery,18 months after surgery,24 months after surgery. To compare the detection results between various time points after surgery and before surgery. Results (1)The levels of serum HE4 and CA125 in the observation group were significantly higher than these in the control group,difference was highly statistical significance(P 〈 0.01).(2)At 1 month,3 months,6 months,12 months after surgery,the levels of serum HE4 and CA125 of the observation group were gradually decreased.Compared with preoperative,the differences were highly statistical significance(P 〈 0.01).To compare the detection results at the four time points with results of the previous time point,the difference were highly statistical significance(P 〈 0.01). After 18 months and 24 months,the levels of serum HE4 and CA125 were increased again by a certain extent,to compare the detection results at the two time points with results of the previous time point,but the difference were not statistically significant(P 〉 0.05 ).(3)In the observation group,9 patients had relapsed at 12 months after surgery. At 18 months and 24 months after surgery,the levels of serum HE4 and CA125 of recurrence were rised again,relapse patients(n=9)compared with the no recurrences ( n=34 ), the difference were highly statistical significance(P 〈 0.01). Conclusion Joint detection of serum HE4 and CA125 can be used as an important means of early screening,surgical treatment evaluation and recurrence monitoring of the ovarian cancer.
