超声新技术对子宫内膜癌及浸润程度的诊断价值 被引量:8

THE NEW ULTRASONIC TECHNIQUE IN THE DIAGNOSIS OF ENDOMETRIAL CARCINOMA AND TO ASSESS THE DEGREE OF INFILTRATION

摘要 目的探讨超声新技术诊断子宫内膜癌及判断肌层浸润深度的价值。方法收集2010年1月~2014年3月在我院经手术治疗的子宫内膜癌患者150例。全部病例术前均应用经阴道实时三维超声多平面成像等超声新技术诊断子宫内膜癌及肌层浸润深度评估(3-DTVS组);其中,45例经核磁共振(MRI)检查(MRI组),以术后病理结果为标准,判断两组诊断子宫内膜癌及其肌层浸润情况的符合率。另选经病理证实的50例子宫内膜良性病变者作三维能量多普勒血管成像(3D—CPA)对照(对照组)。结果经3-DTVS诊断子宫内膜癌的符合率为90.7%,判断肌层浸润的符合率为78.3%。应用3-DTVS与MRI诊断子宫内膜癌及肌层浸润准确率两组间无显着性差异(p〉0.05)。良恶性组比较,3-DTVS组血管成像表现为癌灶区血管异常丰富,不规则,走行紊乱,呈线球样或草团征。VI值在恶性组与良性组比较有显著差异(p〈0.01),FI、VFI值在恶性组与良性组比较有差异,(均p〈0.05)。结论超声新技术对诊断子宫内膜癌及肌层浸润深度有重要临床价值,3D—CPA是一项无创的、定量的检查病灶血管的新方法和指标。 Objective To evaluate the new technique in the diagnosis of endometrial carcinoma and myometrial invasion depth value. Methods From 2010 January to 2014 March in our hospital after operation for treatment of 150 cases of endometrial cancer patients. All eases were applied in endometrial carcinoma and muscular layer infiltration depth assessment of new technology of ultrasonic diagnosis (3 DTVS group) ; 45 cases by nuclear magnetic resonance (MRI) examination as control group ( group MRI), on the basis of the postoperative pathological results as the standard, the judgment of two groups of diagnosis in endometriul carcinoma and its muscular layer infiltration condition coincidence rate. Select the benign lesions in 50 patients with endometrial pathology as a 3 DTV angiography control. Results After 3 DTVS in the diagnosis of endometrial cancer with the rate of 90.7%, the assessment of myometrial invasion were 78.3%. Application of 3 DTVS and MRI in the diagnosis of carcinoma of endometrium and myometrium invasion aceuraey rate between the two groups no significant difference (p 〉 0. 05 ). Conclusion Ultrasonic technology to the diagnosis of endometrial carcinoma and muscle layer infiltration depth has important clinical value. 3D -CPA is a new method and index of vascular lesions in a noninvasive, quantitative.
