摘要 目的 探讨CA125和Ki67在卵巢上皮性癌组织中的表达及与卵巢上皮性癌临床病理学特征的关系,探讨两者之间的相关性,并与血清中CA125的表达进行分析比较.方法 采用免疫组织化学SP法检测CA125和Ki6在58例卵巢上皮性癌组织中的表达,用电化学发光法检测血清中CA125的水平.结果 CA125在卵巢浆乳癌、黏液性癌、透明细胞癌及内膜样癌中的阳性率分别为84.62%、33.33%、66.67%和71.43%,浆乳癌的阳性表达率最高,黏液性癌的阳性表达率最低,浆乳癌vs黏液性癌,差异有统计学意义(P<0.01);CA125在卵巢上皮癌中-低分化组的阳性率(80.00%)高于高分化组(53.85%),但无统计学意义(P>0.05);晚期患者(Ⅲ~Ⅳ期)的阳性率(85.71%)显著高于早期患者(Ⅰ~Ⅱ期)(56.52%)(P<0.05).Ki67在浆乳癌、黏液性癌、透明细胞癌及内膜样癌中的高表达率分别为76.92%、77.78%、100%和71.43%,差异无统计学意义(P>0.05);Ki67在卵巢上皮癌中-低分化组的高表达率(86.67%)显著高于高分化组(46.15%)(P< 0.01);在晚期患者(Ⅲ~Ⅳ期)的高表达率(88.57%)显著高于早期患者(Ⅰ~Ⅱ期)(60.87%)(P<0.05).在卵巢浆乳癌患者组织中CA125和Ki67之间的表达呈正相关(P<0.01).在卵巢浆乳癌患者血清中的CA125浓度与组织中CA125和Ki67的表达呈正相关(P<0.05).结论 CA125和Ki67与卵巢上皮性癌的分化、分级、转移密切相关,卵巢浆乳癌的血清CA125水平与组织的CA125和Ki67表达密切相关,联合检测浆乳癌组织CA125和Ki67的表达水平及血清CA125的浓度,对评估卵巢浆乳癌患者的恶性程度、转移等生物学行为及预后较单独检测更具有实际意义. Objective To investigate the expression of CA 125 and Ki67 in ovarian epithelial carcinoma and its relationship with clinical pathological features of ovarian epithelial carcinoma,to explore the correlation between the two and carry on the analysis and comparison with serum CAl25 expression.Methods Used immunohistochemical SP method to detect CA125 and Ki67 expression in 58 cases of ovarian epithelial carcinoma,electricity chemiluminescence to detect serum CA125 level.Results The positive rate of CA125 in ovary pulp breast cancer,ovarian mucous carcinoma,clear cell carcinoma and endometrial carcinoma were 84.62%,33.33%,66.67% and 71.43% respectively.The positive rate of CA125 in ovary pulp breast cancer was the highest,the lowest in mucous carcinoma,with statistically significant difference (P 〈 0.01).The positive rate of CA125 in moderately and poorly differentiated epithelial ovarian cancer was higher than that in well differentiated epithelial ovarian cancer (80.00% vs.53.85%),without statistically significant difference (P 〉 0.05).The positive rate of CA125 in advanced (Ⅲ-Ⅳ) patients was significantly higher than that in early phase (Ⅰ-Ⅱ) patients (85.71% vs.56.52%; P 〈 0.05).The high expression rate of Ki67 in ovary pulp breast cancer,ovarian mucous carcinoma,clear cell carcinoma and endometrial carcinona were 76.92%,77.78%,100% and 71.43% respectively,without statistically significant difference between them (P 〉 0.05).The high expression rate of Ki67 in moderately and poorly differentiated epithelial ovarian cancer was significantly higher than that in well differeutiated epithelial ovarian cancer (86.67% vs.46.15%; P 〈 0.01).The high expression rate of Ki67 in advanced (Ⅲ-Ⅳ) patients was significantly higher than that in early phase (Ⅰ-Ⅱ) patients (88.57% vs.60.87%; P 〈 0.05).The expression of CA125 and Ki67 in ovarian pulp breast cancer tissue were positively correlated (P 〈 0.01).The serum CA125 level and the expression of CA125 and Ki67 in ovarian pulp breast cancer tissue were positively correlated (P 〈 0.05).Conclusions CA125 and Ki67 are closely related to differentiation,classification,transfer of ovarian epithelial carcinoma.The level of serum CA 125 is closely associated with CA125 and Ki67 expression in ovarian pulp breast cancer tissue.Joint detection of CA125 and Ki67 expression level and the concentration of serum CA125 is more helpful to evaluation of the malignant degree,metastasis and other biological behaviors,prognosis than separate detection.
