化疗联合心理治疗对胃癌化疗患者生存质量及其免疫功能的影响

The Effect on Chemotherapy Combined with Psychotherapy for Gastric Cancer Patients Life Quality and Immune Function

目的探讨化疗联合心理治疗对晚期胃癌患者生存质量及免疫功能的影响。方法将62例晚期胃癌患者随机分为观察组(31例)和对照组(31例),对照组患者采用FOLFOX方案常规化疗。观察组患者在对照组化疗方案基础上,加用心理治疗。3个周期化疗后进行疗效评价。采用自测健康评定量表(SRHMS)评价两组生存质量评分,根据疗效标准(RECIST)评价两组临床疗效;同时对患者取脐静脉血测定全血白细胞和单抗桥联酶标法测定T细胞亚群(CD3、CD4)。结果观察组有效率为68.75%,明显高于对照组(43.3%)(χ2=4.07,P<0.05)。观察组和对照组治疗前SRHMS总分分别为259.55±59.37和259.55±59.37分,两组比较差异无显著性(t=0.06,P>0.05);治疗后,观察组和对照组SRHMS总分分别为347.63±65.16分和288.45±68.19分,两组差异具有显著性(t=3.49,P<0.01);观察组CD4/CD8比值为1.62±0.04,明显高于对照组1.12±0.07,差异具有显著性。结论心理联合化疗能够有效提高胃癌患者化疗疗效、提高其生存质量,增强机体免疫力,建议在胃癌化疗时配合心理治疗。

Objective To explore the effect that chemotherapy combined with psychotherapy for gastric cancer patients life quality and immune function.Methods 62 gastric cancer patients were randomly divided into observation group（31 cases） and control group（31 cases）,The control group were treated by standard chemotherapy FOLFOX scheme,Observation group were treated chemotherapy combined with psychotherapy.Chemotherapy curative effect were evaluation after the cycles of chemotherapy.Before and after therapy,the life quality of the two groups was evaluated with SRHMS,the clinical effect of the two groups was evaluated by RECIST.At the same time for the patient to take venous blood determination of whole blood leukocytes and single bridging enzyme standard method determination of T cell subsets（CD3,CD4）.Results After therapy,the effective rate of observation group was 68.75%,it was higher than that control group（43.30%,χ^2=4.07,P〈0.05）.Before therapy,the total SRHMS score of the observation group and control group were respectively 259.55±59.37 and 259.55±59.37,there was not significant differences（t=0.06,P〈0.05）,after therapy,the total SRHMS score of the observation group and control group were respectively 347.63±65.16 and 288.45±68.19,there was significant differences（t=3.49,P〈0.01）.Observation Group of CD4/CD8 ratio was 1.62±0.04,significantly higher than that control group（1.12±0.07）,there was significant difference.Conclusion Chemotherapy combined with psychotherapy can effectively improve the gastric cancer patients with chemotherapy curative effect,improve the quality of survival,enhance immunity function,suggested that chemotherapy combined with psychotherapy when treated gastric cancer.
