摘要
目的观察高速电钻磨削及等离子射频处理术后兔下鼻甲的形态学变化,为其临床应用提供实验依据。方法荷兰兔22只,2只作为正常对照组,其余20只随机分为电钻磨削组及等离子射频组,分别于两组动物右下鼻甲行高速电钻磨削或等离子射频手术,左侧下鼻甲仅注射少量生理盐水作为自身对照;分别在术后第1、3、7、14、21天取下鼻甲组织于光镜及扫描电镜观察比较其形态学变化。结果肉眼观察见电钻磨削组下鼻甲缩小较等离子组明显;光镜下见等离子组下鼻甲组织炎症坏死范围较电钻磨削组大、纤维渗出多;电钻磨削能有效去除下鼻甲骨质;电镜下显示两组下甲黏膜纤毛于21天后均修复良好,且以电钻组纤毛损伤区域较局限而术区外纤毛保护良好。结论两种手术方法对兔下鼻甲纤毛上皮均有损伤效应但可恢复;电钻磨削能有效去除骨质并对黏膜纤毛损伤较轻。
Objective To observe mucosal morphological changes of rabbit inferior turbinate following the operation either with high-speed electric drill grinding or plasma radiofrequency ablation to provide experimental data for their clinical practical usage.Methods Two New Zealand rabbits were taken as normal controls and other 20 were randomly divided into two groups, with high speed drill grinding on one group and low temperature plasma radiofrequency ablation on the other group, to operate on the right inferior turbinate and with the other side of inferior turbinate injected only by some volume of normal saline for each group respectively. Then, mucosal samples were taken on the 1, 3, 7, 14 and 21 days respectively after the operation to observe morphological changes in mucosa of inferior turbinate under light microscope and scanning electron microscope.Results As seen by naked eyes, shrunk inferior turbinate was more significant in the samples of electric drill grinding group than that treated by plasma radiofrequency ablation. Under light microscope, it was observed that more obvious inflammatory reaction, necrotic changes and fibrosis presenting in the samples of inferior turbinate in plasma radiofrequency ablation group, but much more bone destruction in high speed electric drill grinding treating group. Under electron microscope, the damaged mucosal cilia of two groups could be seen to recover well. In high speed electric drill grinding group, the cilia outside the operation area were remained better, and the damaged cilia was closely limited into the operating area.Conclusions These two methods of inferior turbinate operation will both cause cilia damage in inferior turbinate mucosa but can recover well. Moreover, the operation with high speed electric drill grinding is able to remove turbinate bone more effectively and protect mucosal cilia surrounding the operation area better.
