The orderly grading service in community health care can ensure the maximum utilization of the general practitioner＇s talent resources, provide timely normative diagnosis and treatment for the patients and ease the difficulty in medical care. The article explores how to apply the differentiated management mode in communities with the hope of realizing the two - way referral, improving the treatment rate in community medical centers and the rates of standardized management and disease control for chronic diseases, reducing the chronic disease incidences, increasing satisfaction degree of the community residents and final- ly providing the related experiences for healthcare reform.
