肱骨大结节骨折的个体化手术

Individualized surgical treatment of humerus greater tuberosity fracture

目的探讨肱骨大结节骨折的个体化治疗方法和效果。方法 2009年5月-2013年3月本院对87例肱骨大结节骨折分别采取切开复位可吸收螺钉内固定及可吸收螺钉结合不吸收线张力带内固定或带缝线锚钉、解剖钢板固定,观察临床效果。结果本组87例患者均获门诊随访,肩关节功能按照Neer评分方法,吸收螺钉内固定病例优良率和可吸收螺钉结合不吸收线张力带内固定或带缝线锚钉病例优良率为91.17%,解剖钢板固定病例优良率为96.22%两手术方法治疗组比较无统计学意义(P>0.05)。结论对于移位的肱骨大结节骨折主张个体化手术治疗,能较大程度让患者获得一个无痛的、活动范围正常或接近正常的肩关节。

Objective To explore the individualized surgical treatment of humerus greater tuberosity fracture and observe its clinical effects .Methods 87 cases of humerus greater tuberosity fracture in our hospital from May 2009 to March 2013 were individually treated.Measures taken were：1.open reduction and internal fixation with absorbable screw combined with ab-sorbable screw with non absorbable suture tension band fixation or suture anchors ,and 2.anatomic plate fixation .Different re-sults were achieved .Results All the 87 patients were followed up .Shoulder function was evaluated according to the Neer sco-ring system.The excellent rate for measure 1 was 84.21%and the excellent rate for measure 2 was 95.59.There was no statis-tical significance（P〉0.05） in comparison between the two treatment groups .Conclusion Individualized surgical treatment is recommended for displaced humerus greater tuberosity fracture , which can allow patients , in a great measure, a painless, nearly normal or even normal shoulder joint .
