子宫缝合术联合欣母沛治疗子宫收缩乏力性产后出血的临床研究

Clinical Observation of Uterine Suture Combining with Carboprost Tromethamine Injection Treatment for Uterine Atony Postpartum Hemorrhage

目的 探讨子宫缝合术联合卡前列素氨丁三醇（欣母沛）治疗子宫收缩乏力性产后出血的临床价值.方法 选择2012年6月至2013年8月江油市第二人民医院收治的子宫收缩乏力性产后出血患者60例,采用随机数字表法分为两组：对照组（30例）行常规子宫缝合术;观察组（30例）行子宫缝合术联合欣母沛治疗.观察两组患者的雌二醇、促卵泡刺激素（FSH）、潮热评分、子宫内膜厚度及出血时间（≤2 h,＞2 h）、出血量（500～1000 mL,≥1000 mL）的比例.结果 观察组雌二醇水平高于对照组,子宫内膜厚度大于对照组,FSH、潮热评分均小于对照组,差异有统计学意义（P＜0.05）.观察组患者出血时间≤2 h和出血量为500～1000 mL的比例显著高于对照组,差异有统计学意义（P＜0.05）.结论 子宫缝合术联合欣母沛可以更好地改善出血产妇临床症状,减少出血量和出血时间,可以进一步推广该种治疗方式的运用,以提升医疗服务水平.

Objective To explore the clinical value of uterine suture combined with carboprost tromethamine on uterine atony postpartum hemorrhage.Methods A total of 60 cases of uterine atony postpartun hemorrhage from Jiangyou Second People＇s Hospital during Jun.2012 and Aug.2013were randomly divided into two groups,control group（30 cases） received routine uterine suture,observation group（30 cases）received uterine suture combined with carboprost tromethamine treatment.The estradiol,follicle-stimulating hormone（FSH）,hot flashes score,endometrial thickness and proportions of bleeding time（≤ 2 h,〉 2 h）,bleeding amount（500-1000 mL,≥ 1000 mL） of the two groups were observed.Results The level of estradiol of the observation group was higher than the control group,the endometrial thickness bigger than the control group,FSH,hot flash scores were lower than the control group,with statistically significant differences（P 〈 0.05）.The proportion of bleeding time ≤2 h and the bleeding amount of 500-1000 mL of the observation group was significantly higher than the control group,with statistically significant difference（P 〈 0.05）.Conclusion The use of uterine suture combined with carhoprost tromethamine on uterine atony postpartum hemorrhage,can effectively improve the clinical symptoms,and reduce the bleeding amount and time,further promotion of the treatment should be done to improve the medical service level.
