白内障手术并发症应用玻璃体手术治疗的临床研究

Clinical outcomes of vitrectomy in treatment for complications caused by cataract surgery

目的分析总结玻璃体切除术在白内障术中及术后并发症中的应用时机及价值。方法对2004年1月至2013年10月由本文第一作者接诊的81例(81只眼)白内障术中、术后并发症患者适时进行玻璃体切除术,其中包括术后化脓性细菌性眼内炎19例、术中暴发性脉络膜上腔出血4例、术中晶状体后囊破裂导致晶状体核及皮质坠入玻璃体腔28例、人工晶状体脱位于玻璃体腔9例、视网膜脱离21例。结果随访1个月至10年,所有患者适时的进行玻璃体手术后,白内障手术并发症均得到有效控制,大部分患者视力有不同程度提高。结论白内障术中、术后出现的严重并发症及时发现并正确处理,适时行玻璃体切除术是目前最为有效的治疗方法。

Objective To study the timing and clinical outcomes of vitrectomy for serious complications during and after cataract surgery .Methods We retrospectively reviewed 81 patients who were consulted and treated with vitrectomy from January 2004 to October 2013 for complications from previous cataract surgeries .These serious complications included bacterial ophthalmoblennorrhea in 19 cases, suprachoroidal hemmarrage during operation in 4 cases, lens nucleus and cor-tex dropping into vitreous due to posterior capsule rupture in 28 cases, intraocular lens dropping into vitreous in 9 cases, and retinal detachment in 21 cases.Results Treatment with vitrectomy was effective in all patients .Follow-up period ranged from 1 month to 10 years.Various degrees of improvement in visual acuity were accomplished in these cases .Con-clusion Prompt vitrectomy is the most effective treatment for serious complications caused by cataract surgery .
