65例阴道试产中转剖宫产临床观察

Clinical Observation of 65 Cases with Vaginal Trial Production Transfer Caesarean Delivery

目的探讨分析导致临床上阴道试产中转剖宫产的原因以及相关性影响因素。方法以在我院妇产科生产且均为阴道试产中转剖宫产的65例产妇以及54例顺产产妇作为研究对象,将其分为观察组和对照组,观察组65例,对照组54例,对所有产妇的病历资料均采用回顾性的方法进行分析,了解产妇阴道试产中转剖宫产的相关因素。结果对比两组患者的情况,以上65例阴道试产产妇在生产过程中改为剖宫产的影响因素主要包括了产妇的成产次数、年龄、胎儿体重、产程停滞、产程延长、胎儿窘迫、疤痕子宫、产妇耐受力差、头盆不称、助产人员技术水平问题以及妊娠期合并症等。结论产妇在生产之前,必须对头盆的关系进行准确地评估,同时对产程进行严密的观察,对产程中发生的异常情况进行综合性处理,对阴道试产以及剖宫产的指征进行全面的掌握,这样能够将产中中转的几率大大降低,促进产科并发症的降低。

Objective To study and analyze the cause of clinical vaginal trial production transfer cesarean delivery and the correlation factors. Methods Selecting 65 cases with vaginal trial production transfer cesarean delivery and 54 eutocia puerperas during 2012-01-01～ 2013-01-01 as the research subjects,and dividing them into the observation group and the control group. 65 cases were in the observation group and 54 cases were in the control group. Retrospective method was used to analyze the puerperas＇ medical record to understand the related factors of aginal trial production transfer cesarean delivery. Results Through the comparison of the patients in the two groups,the main influence factors of the 65 women vaginal trial production in the process of production to cesarean section include the maternal number,age,fetal weight,stagnation of labor,prolonged labor,fetal distress,scar uterus,maternal tolerance,poor head basin,the problems of midwives technology level and pregnancy complications,etc. Conclusion Before maternal production,the relationship adversary basin must be accurately evaluated,at the same time observation of labor should be restricted,to comprehensively grasp the indications of vaginal trial production and cesarean delivery,to reduce the transfer rate,and to decrease obstetric complications.
