U波异常在诊断缺血性心脏病中的价值研究 被引量:2

摘要 目的分析心电图U波异常在诊断缺血性心脏病中的应用价值。方法对有心绞痛等症状的疑似冠心病患者60例患者分别行心电图、运动负荷实验和冠状动脉造影DSA检查,比较ST段下移和U波异常诊断缺血性心脏病灵敏度和特异度差异。结果 60例疑似冠心病患者中42例DSA检查阳性。对42例阳性患者普通心电图检查,其中ST段下移阳性和U波异常者分别为19例和10例,计算其灵敏度分别为45.24%和23.81%;对42例阳性患者运动负荷试验后心电图检查,其中ST段下移阳性和U波异常者分别为21例和14例,计算其灵敏度分别为50.00%和33.33%。对18例DSA检查阴性患者普通心电图检查,其中无ST段下移和U波正常者分别为7例和15例,计算其灵敏度分别为38.89%和83.33%;对18例DSA检查阴性患者运动负荷试验后心电图检查,其中无ST段下移和U波正常者分别为8例和17例,计算其灵敏度分别为44.44%和94.44%。结论 U波异常诊断缺血性心脏病敏感性不如ST下移高,但其优点在于特异性远高于ST下移,特别是运动后U波异常者更应注意缺血性心脏病的可能。
