人工全膝关节置换术治疗要点分析

The analysis of operative key points on total knee arthroplasty

目的探讨终末期膝关节疾病行人工全膝关节置换术的手术要点。方法回顾性分析行人工全膝关节置换术213例(238膝),分析其手术操作技巧、术后镇痛方法、患者依从性等与关节功能恢复情况的关系。结果 168例(170膝)获得3个月至4.5年随访,感染3例(4膝);术前膝关节HSS评分为(32.45±2.55)分,术后为(78.45±3.65)分,达到优良级别。结论人工全膝关节置换术是治疗终末期膝关节疾病的一种有效方法,手术效果与手术技巧、术后早期镇痛、患者的依从性等关系密切。

Objective To investigate the operative key points on total knee arthroplasty for end-stage disease of the knee. Method The total knee replacement patients of 213 in 238 cases were analyzed retrospectively. To analyzed the relationship between recovery of joint function and surgical techniques,postoperative analgesic method and the compliance of patients. Result 168 patients including 170 knees were followed up for 3 months to 4. 5 years. The 3 case including 4 knees were infectived. All the followed HSS ware （32. 45±2. 55） in preoperative and （78. 45± 3. 65） in postoperative. All the knee deformity were corrected significantly. Conclusion Total knee arthroplasty is an effective treatment for end-stage disease in the knee. The effect of surgery dependent on surgical techniques,early postoperative analgesia and patient compliance.
