摘要
目的探讨长期输血患者体内铁代谢与铁调素Hepcidin表达水平的相关性,为临床诊疗和合理安全用血提供依据。方法抽取本院收治的30例肿瘤患者和23例β地中海贫血患者,在其累计输血(RBC)>20 U或10 U(儿童)后抽取静脉血标本进行铁代谢指标(全血铁、转铁蛋白和铁蛋白)与铁调素Hepcidin水平检测,并对检测数据进行相关性分析。结果长期输血导致铁代谢紊乱,患者体内显示铁负荷超载;同时Hepcidin表达水平上升,分析显示不同病种某些铁代谢指标与其存在相关性。结论长期输血患者应充分考虑其体内铁代谢状况,适时做出调节,以达到输血与治疗效果的合理平衡。
Objective To explore the correlation between iron metabolism and expression level of hepcidin in long- term transfusion patients aiming at providing references for clinical diagnosis and treatment, and blood transfusion in safe and reasonable way. Methods Thirty patients with tumor and 23 patients with β-thalassemia admitted into our hospital were selected.After RBC in the accumulative blood transfusion being more than 20 U or 10 U in child,the indexes of iron metabolism like iron in while blood, transferrin, and ferroprotein and expression level of hepcidin were tested by drawing venous blood.The tested data were carried on correlation analysis. Results Long-term blood transfusion led to disorder of iron metabolism displaying iron overload in the body as well as increasing expression level of hepeidin.According to outcomes of analysis,some indexes of iron metabolism in different diseases had statistical correlations with the expression level of hepcidin. Conclusion For long-term blood transfusion patient,it is proper to take the state of iron metabolism in vivo into consideration and make some adjustment in due time so as to realize a reasonable balance between blood transfusion and therapeutic effect.
