青岛地区丙型肝炎基因分型的研究

GENOTYPING STUDY ON HEPATITIS C IN QINGDAO AREA

目的探讨青岛地区丙型肝炎基因型的分布状况,发现丙型肝炎病毒(HCV)流行优势株,分析丙型肝炎基因型与病人性别、感染途径、炎症活动度、病毒载量等的相关性。方法应用RT-PCR方法,对46例青岛地区丙型肝炎病人的血清RNA提取物NS5B区和(或)CORE-E1区进行扩增,扩增片段进行基因测序,根据测序结果在PUBMED网站中获得基因分型结果,分析不同丙型肝炎基因型与病人性别、感染途径、肝病程度、HCV RNA含量及谷丙转氨酶(ALT)、谷草转氨酶(AST)水平的关系。结果 46例血清标本,HCV RNA均为阳性,44例可以分型,分型率为95.65%,其中基因1b型28例(63.64%),2a型14例(31.81%),3b型2例(4.55%)。丙型肝炎基因型的分布与病人性别、感染途径、肝病程度、HCV RNA含量及ALT、AST水平均无关(P>0.05)。结论青岛地区HCV流行株为基因1b型和2a型,基因1b型为优势株,同时发现3b基因型。基因型与丙型肝炎的疾病进展程度及炎症活动度无关。

Objective To investigate the distribution of genotypes of hepatitis C in Qingdao area, detect the prevailing strains of hepatitis C virus （HCV）, and analyze the correlation of genotype of hepatitis C with patient’s sex, route of infection, activity of inflammation and viral load. Methods Using reverse-transcription polymerase chain reaction （RT-PCR） assay, the NS5B or CORE-E1 region of extract from serum RNA of 46 cases of hepatitis C was amplified, gene sequencing was then carried out for the amplified fragments. According to the results of the sequencing, ther results of genotyping were obtained from PUBMED web site. The relationship between different genotypes of hepatitis C and patient’s sex, route of infection, the degree of the disease, the content of HCV RNA, levels of ALT and AST were analyzed. Results Of the 46 serum samples, all were HCV RNA positive, 44 of which could be grouped, grouping rate being 95.65%. Of which, 28 （63.64%） cases were genotype 1b, 14（31.81%） cases were genotype 2a, and two （4.55%）cases were genotype 3b. No relationship was observed between genotype distribution and patient’s sex, route of infection, the degree of the disease,the content of HCV RNA, levels of ALT and AST. Conclusion The prevalent HCV strains in Qingdao area were genotypes 1b and 2a with1b being dominanted. HCV genotype 3b was found at the same time. The genotype was not correlated with the degree of progression of the disease and the activity of the inflammation.
