广西五城镇女性性工作者阴道灌洗与安全套使用行为相关性分析

A study on association between vaginal douching and condom use behaviors among female sex workers in five towns of Guangxi

目的了解广西五城镇（PX、DA、BZ、LT、BS）女性性工作者阴道灌洗行为的现况,及其与安全套使用行为的关系。方法在五城镇性服务场所,用方便抽样招募女性性工作者（FSW）进行问卷调查,收集FSW阴道灌洗行为、性行为及其他相关信息;用多因素分析方法探讨阴道灌洗频率与安全套使用比例之间的关系。结果五城镇共905名FSW参加调查,平均年龄为31.4-34.7岁。各现场FSW曾灌洗阴道的比例范围为74.8%-90.0%;近30天灌洗过阴道的比例为38.4%-84.2%,较频繁灌洗阴道（≥4次/月）的比例为15.2%-58.6%。各现场近30天有灌洗阴道者为18.9%-49.1%,平均每天至少灌洗一次。五城镇近30天与客人发生性行为均使用安全套的FSW的比例为66.1%-91.4%。多因素分析结果显示：PX的FSW较频繁灌洗阴道的比例低于其余现场约2-7倍;相对于近30天性行为中未用安全套的妇女,每次性行为均用安全套的妇女偶尔灌洗阴道的比例增加1.6倍[95%可信区间（CI）：1.09-7.76],较频繁灌洗阴道的比例增加2.8倍（95%CI：1.09-6.26）。结论五城镇FSW阴道灌洗行为流行率较高,但城镇间存在一定差异。使用安全套的FSW灌洗阴道的比例更高。

Objective To investigate vaginal douching behavior and its relationship with condom use among female sex workers （FSWs） in five towns of Guangxi. Methods A convenience sampling was used to recruit FSWs in five towns and a face-to-face questionnaire-based interview was employed to collect data about vaginal douching, sex behavior and other relevant information. Multivariate analysis was used to analyze association between vaginal douc hing and condom use behavior. Results A total of 905 eligible women from five towns were interviewed. The average ages of the participants in each town were between 31.4 and 34.7 years old. of Women reported vaginal douc- hing accounted for 74.8%-90.0%. In the last 30 days, 38.4%-84.2% had douched, 15.2%-58.6% had douched 4 times or more and 18.9%-49.1% douched at least 1 time per day on average. In the past 30 days, 66.1%-91.4% of the participants of each town reported having consistently used condoms with their clients. Multinominal logistic regression showed that women from PX 2-7 times less likely frequently douched than those from other towns. Compared to the women who had never used condoms in the past 30 days, women using condoms consistently were more likely to have occasional douche （aOR= 1.6,95 %CI;1. 097.76） or frequent douche （aOR =2.8,95%CI 1.09-6.26）. Conclusions Vaginal douching was prevalent among FSWs of the five towns, but varied in different places. FSWs using condoms were more likely to douche.
