摘要
目的探讨经颅超声检查(TCS)对多系统萎缩(MSA)的诊断价值。方法采用GE Vivid7超声诊断仪对21例MSA患者和20例帕金森病(PD)患者进行TCS检查。检测、评定黑质的回声强度(分为I^V级,≥Ⅲ级为强回声),测量双侧黑质强回声的面积,计算其强回声面积与中脑总面积(S/M)的比值。结果MSA组黑质强回声的比率(7例,33.3%)显著低于PD组(13例,65%)(χ2=4.11,P<0.05)。并且两组黑质回声各级分布的差异有统计学意义(P<0.005),而两组间黑质强回声面积和S/M比值的差异均无统计学意义(均P>0.05)。结论 TCS检查显示MSA患者出现黑质强回声的较少,TSC检查对MSA与PD的鉴别诊断有一定的价值。
Objective To explore the diagnostic value of transcraniai sonography ( TCS ) on multiple system atrophy(MSA).Methods TCS examination was performed by GE vivid7 ultrasonic diagnostic system in 21 MSA patients and 20 Parkinson disease ( PD) patients.The echo signal intensity of the substantia nigra was detected and evaluated(the level was classified into grade I-V,the echo intensity grade ≥Ⅲ was deemed as hyperechogenicity ). The area of the bilateral substantia nigra hyperechogenicity was measured ,and the ratio of its hyperechogenicity area and whole midbrain ( S/M ) area was calculated .Results The ratio of substantia nigra hyperechogenicity in MSA group(33.3%,7/21) was significantly lower than that in PD group (65.0%,13/20)(P〈0.05).The difference of distribution of echo intensity grade in substantia nigra had statistically significant between MSA group and PD group (P〈0.005), while the differences of the area of the substantia nigra hyperechogenicity and the ratio of S /M had no statistically significant between MSA group and PD group ( all P 〉0.05 ) .Conclusions The substantia nigra hyperechogenicity showed by TCS is relatively less in MSA patients .TCS has certain value to distinguish MSA and PD.
