中西医结合治疗带状疱疹后神经痛的临床观察

Clinical Observation on The treatment of Post-herpetic Neuralgia With combination of TCM and Western Medicine

目的观察中西医结合方法(电针、穴位注射腺苷钴胺、口服加巴喷丁)治疗带状疱疹后神经痛的临床疗效,并对比口服腺苷钴胺联合加巴喷丁治疗的疗效差异及不良反应。方法将117例带状疱疹后神经痛患者随机分为2组,观察组59例采用电针、穴位注射腺苷钴胺、口服加巴喷丁治疗,对照组58例采用口服腺苷钴胺联合加巴喷丁治疗,疗程均为5周,随诊3个月观察患者临床疗效及药物不良反应发生率。结果观察组总有效率92%,不良反应发生率10%;对照组总有效率78%,不良反应发生率10%。2组总有效率比较有显著性差异(P<0.05),不良反应发生率比较无显著性差异(P>0.05)。结论电针、穴位注射腺苷钴胺、口服加巴喷丁治疗带状疱疹后神经痛疗效优于口服腺苷钴胺联合加巴喷丁治疗,而且未增加不良反应发生率。

Objective It is to observe the clinical curative effect of integrated traditional Chinese and Western Medicine （acupuncture, acupoint injection of cobamamide, oral gabapentin） in treatment of postherpetic neuralgia, and compare the ef- ficacy differences among oral cobamamide combined with gabapentin treatment and adverse reaction. Methods 117 patients with postherpetic neuralgia were randomly divided into observation group and control group, 59 cases in observation group were treated with electro acupuncture, acupoint injection of cobamamide and oral gabapentin, 58 cases in control group were treated with oral administration of cobamamide combined with gabapentin, the course of treatment was 5 - week, and follow-up for 3 - month, the clinical curative effect and adverse reaction on the incidence of drug was observed. Results The total effective rate and the adverse reaction rate was 91.53% and 10.17% respective in observation group, and was 77.59% and 10.34% in control group, there was significant difference （ all P 〈 0.05 ） in total effective rate and no significant difference in the inci- dence of adverse reactions （ P 〈 0.05） between both groups. Conclusion The Effect of acupuncture, acupoint injection of co- bamamide, oral gabapentin in the treatment of postherpetic neuralgia is better than that of oral administration of cobamamide combined with gabapentin, and without increasing the incidence of adverse reaction.
