凝血酶原复合物治疗体外循环术后渗血过多的临床观察

Clinical analysis of prothrombin complex concentrate for treating more oozing of blood after open-heart operations under extracorporeal circulation

目的 :寻找治疗体外循环术后渗血过多的有效办法。方法 :随机将体外循环转流时间大于 1 5h患者分成两组 :A组 (实验组 ) 18例 ,术后应用凝血酶原复合物 40 0～ 80 0IU ;B组 (对照组 ) 5 7例 ,术后不用凝血酶原复合物。并记录两组每位患者的总引流量和总输血量。结果 :两组患者的平均总引流量 (A :5 61 3 8± 5 0 5 2ml ;B :73 7 47± 5 3 3 8ml,P <0 0 1)、平均总输血量 (A :175 3 5 9± 110 3 5ml ;B :2 2 87 3 6± 2 0 5 15ml,P <0 0 1)相比较 ,差异均有非常显著性。结论 :凝血酶原复合物可有效减少体外循环术后渗血 ,特别是体外循环转流时间小于 1 5h的患者。

Objective To find out an effective measure to treat more oozing of blood after extracorporeal circulation Methods 75 patients whose operations of extracorporeal circulation would lasted over 1 5 hrs were divided into two groups at random:the Group A(18 patients experimental,groups) would have be given prothrombin complex concentrate (PCC) 400～800 IU besides routine treatment after extracorporeal circulation,while the Group B(57 patients,control group) had only been received routine treatments We had conscientiously recorded the every patient's total capacity for drainage and transfusion Result There were very clear statistical significances between Group A and Group B on the total capacity for drainage (A:561 38±50 52 ml;B:737 47±53 38 ml,P<0 01) and transfusion (A:1 753 59±110 35 ml;B:(2 287 36±205 15 ml,P<0 01) Conclusion The PCC effectively reduce the ozzing after extracorporeal circulation,especially operation of extracorporeal circulation lasted over 1 5 hrs
