摘要
目的 提高尺神经嵌压综合征的早期诊断率 ,指导有效治疗。方法 报告 2 6例尺神经嵌压综合征的临床及电生理资料 ,并与 30例正常人进行了对照研究。结果 显示 2 6例尺神经嵌压综合征有 16例系肘管综合征 ,病侧肘上 肘下段尺神经运动传导速度 (MCV)为 2 0 .1± 13 .5m/s(P <0 .0 1) ,动作电位波型离散、波幅降低 ;尺神经指 -腕段感觉传导速度 (SCV)为 14 .5± 2 1.4m/s(P <0 .0 1) ;外展小指肌及第一骨间肌均呈神经源性损害。 10例系尺管综合征 ,病侧尺神经MCV正常 ;尺神经指 -腕段SCV 40 .1± 16 .8m/s(P <0 .0 5 ) ;第一骨间肌均呈神经源性损害。结论 电生理学检查是诊断尺神经嵌压综合征的可靠手段 ,可早期确诊、准确定位 ,指导临床治疗及评价预后 。
Objective To improve the early diagnosis of the ulnar nerve entrapment syndrome and direct the treatment.Method 26 cases of ulnar entrapment syndrome were reported,and contrasted with 30 cases of healthy subjects.Result There were 16 cases of cubital tunnel syndrome in 26 cases of patients.The ulnar nerve motor conduction velecity(MCV) in the illness arms between proximal and distal to the ulnar groove in the cubital tunnel syndrome was 20.1±13.5m/s(P<0.01),and the ulnar nerve distal snesory conduction velocity(SCV) was 14.5±21.4m/s(P<0.01).EMG showed neurogenic damage in the abductor digiti minimi muscles and the first dorsal interosseous muscles of the 16 patients.There are 10 cases of ulnar tunnel syndrome.The ulnar nerve SCV in the sickness arms was 40.1±16.8m/s(P<0.05),but EMG showed neurogenic damage only in the first dorsal interosseous muscles.Conclusion The electrophysiological examnation is the reliable method in the diagnosis of the ulnar nerve entrapment syndrome.It can help to make a definit diagnosis early,orientate accurately,direct the treatement and supply a dependable basis of differential diagnosis.
