Analysis on the Relationship between Intellectual Impairment and the CT Imaging of Multiple Cerebral Infarction

摘要 目的 探讨多发性脑梗死患者智能障碍与CT上所示梗死部位、梗死容积及其它因素之间的关系。方法 应用韦氏智力量表检测智能障碍的发生与梗死部位、容积等诸多因素的关系。结果  110例患者出现智能障碍 ,其中轻度 41例 ,中度 38例 ,重度 31例。智能障碍与其梗死体积呈明显正相关 ,与各皮层及皮质下萎缩因素呈明显负相关 ,与文化程度呈明显负相关。左侧大脑较右侧大脑病变智能障碍的发生率高。结论 智能障碍的发生与梗死体积、慢性脑循环障碍所致皮层及皮质下病理改变。 Objective In order to explore the correlation between intellectual impairment and computed tomographic changes with infarct location, extent of focal lesions and other factors in multiple cerebral infarct patients. Methods 236 cases of multiple cerebral infarct patients identified by CT findings were studied with the neuropsychological tests by WAIS RC. To analyze the correlation among the occurrence of intellectual impairment and sex,age,education level, the location and extent of focal lesions as the pressense of cerebral atrophy or subcortical degeneration in 110 multiple cerebral infarct dementia index cases and 126 multiple cerebral infarction without dementia index cases. Results 46.6% of them were shown to have intelligence impairment. including light degree 37.3%, moderate 34.6%, severe 28.2%. The intellectual impairment was positively relative to extent of focal lesion. but negatively relative to the cerebral atrophy factor, subcortical degeneration factor and education levels. The result with intellectual impairment of left cerebral infarct was higher than the right. Conclusion It was suggested that a single explanation for multi infarct dementia is not adequate. Rather, multiple factors including extent of focal lesions,pathologic changes of cerebral and subcortical with chronic brain circulation disorder and low educational level,may independently contribute to intellectual impairment.
