深低温体外循环方法对肺功能影响的实验研究 被引量:5

摘要 为研究深低温停循环 ( DHCA)和深低温低流量 ( DHL F)两种体外循环方法对肺功能的影响。采用幼猪模拟临床复制出 DHCA和 DHL F模型 ,于转流前、转流结束和停体外循环后 1 h等 3个时点测定肺顺应性、气道阻力、动脉—肺泡氧分压比率、肺血管阻力指数和中性粒细胞滞留率等指标值。结果表明 DHL F后 ,肺顺应性和动脉—肺泡氧分压比率的下降程度 ,肺血管阻力指数和中性粒细胞滞留率的增加程度均明显高于 DHCA。两组气道阻力无差别。结果提示 ,DHL F较之 s:The influence of Deep Hypothermic Circulatory Arrest (DHCA) and Deep Hypothermic low flow cardiopulmonary bypass (DHLF) on lung was studied. After establishment of DHCA and DHLF model on piglets according to clinical cardiovascular surgery, measurements of lung compliance?airway resistance?artery alveolar O 2 pressure ratio?pulmonary vascular resistance and neutrophil sequestration ratio were detected at 3 different time,i.e. before institution of Cardiopulmonary bypass(CPB)?at the end of CPB and 1 hour after cessation of CPB.The results showed that more severe decrement of lung compliance and artery alveolar O 2 pressure ratio and greater increment of pulmonary vascular resistance and neutrophil sequestration ratio were observed in DHLF than in DHCA. No difference were observed on increments of airway resistance between the two groups. It was concluded that DHLF could cause more severe pulmonary injury than DHCA on piglets lung.
