摘要
目的探讨一侧大脑皮质病变对知觉消去的影响及其临床意义和理论机制。方法采用简单刺激法和改良 QET法。结果改良 QET法阳性率明显高于简单刺激法;左皮质病变时,可有对侧和同侧消去阳性;左额叶病变时易出现同侧消去阳性。结论改良 QET法比简单刺激法敏感;用改良 QET法揭示了知觉消去存在两个不同水平的竞争和抑制现象,知觉消去的机制可能与两侧大脑半球的竞争和优势抑制及两侧大脑半球间的联络纤维有关;左顶叶病变阳性率最高。
Objective To study the effect and clinical meaning and pathogenic of extinction on cerebral cortex lesion.Methods Tactile extinction to classical extinction test(simple stimuli)and modified Quality extinction test(QET)were used. Results The positive rate of extinction using modified QET was higher than that of simple stimuli;left cortex lesion could result in extinction both in the contralateral side and the ipsilateral side;left frontal sites lesion caused ipsilateral extinction. Conclusion:The modified QET was more sensitive than simple stimuli;modified QET showed extinction has competiting and restricting effect at 2 different levels,the mechanism of extinction is correlated to competition and adventage restraint of diplo- hand cerebral hemisphere and liaise correliation fiber of diplo- hand cerebral hemisphere;the highesttthe positive rate of extinction was observed in left parietal lesions.
