肾综合征出血热患者早期尿细胞融合程度与病情及预后的关系 被引量:1

Relationship of urine cells fusion index with renal lesion and prognosis in the patients with hemorrhagic fever with renal syndrome

摘要 目的 为了探讨肾综合征出血热 (HFRS)患者早期尿细胞融合程度与临床病情预后的关系。方法  1.将 15 5例患者 7病日以内的尿细胞融合程度按有关公式换算为融合指数并与其血清中肌酐、尿素氮水平进行相关性分析 ;2 .把患者按照尿蛋白量 +、+ +、+ + +、+ + + +分为 4组 ,比较组间早期尿细胞融合指数的差别 ;3 .将患者分为轻、中、重、危重型等四型 ,比较型间早期尿细胞融合指数的差别。统计学方法使用SAS软件。结果  1.血清肌酐、尿素氮值与早期尿细胞融合指数呈正相关 ,相关系数r值分别为 0 .9340 (P <0 .0 0 0 1)、0 .82 0 4(P <0 .0 0 0 1) ;2 .不同尿蛋白量组间早期尿细胞融合指数有较明显差别。 4组中 ,+ + + +组尿细胞融合指数高于其它 3组 ,F =13.5 3(P <0 .0 0 0 1) ,等级相关系数rs=0 .4783(P <0 .0 0 0 1) ;3 .不同型别间尿细胞融合指数有明显差别。轻、中、重、危重型尿细胞融合指数依次增高 ,F =93 .6 8(P <0 .0 0 0 1) ,等级相关系数rs=0 .6 2 83(P <0 .0 0 0 1)。 Objective To study the relationship of fusion cells in the urine to renal lesion or prognosis of hemorrhagic fever with renal syndrome (HFRS). Methods In 155 cases of HFRS, the fusion index (FI) of the fusion cells in the urine were calculated during the early stage of the disease (<7 days). The FI and sera creatinine (Cr) or blood urea nitrogen (BUN) were analyzed by the analysis of correlation. According to urine protein of +~++++, the 155 cases were divided into 4 groups. The variance of the FI of the 4 groups were analyzed by the analysis of variance and all cases were devided into mild, moderate, severe and critical clinic types too. The variance of the FI in those types were analyzed as well. The statistics was done by microsoftware of Statistics Analysis System(SAS). Results The FI were analyzed to be a linear positive correlation to Cr ( r =0.9340 P <0.0001) and BUN ( r =0.8204 P <0.0001). By the analysis of variance, the variance of FI in 4 groups were significant ( F =13.53 Spearman correlation coefficient r s =0.4783 P <0.0001) and the variance of FI in different types were significant as well ( F =93.68, r s =0.6283 P <0.0001). Conclusion Urine cell fusion index is closely related to the renal lesion and the prognosis in the patients with HFRS.
