摘要
目的 探讨巩膜阶梯状切口对人工晶状体植入术后角膜散光的影响。方法 对 43眼采用巩膜阶梯状切口白内障囊外摘出及人工晶状体植入术 ,术后早期以角膜曲率计测量及验光观察角膜散光的变化 ,并随机与常规的角膜缘切口对照。结果 巩膜阶梯状切口组术后 3天平均角膜散光 (3 0 2± 1 6 2 )D ,术后 3月 (1 2 1± 0 98)D。对照组 (4 78± 2 2 1)D和(2 99± 1 36 )D。 2组比较 (t检验 ,P <0 0 5 )。术后 1周裸眼视力≥ 0 5者 35眼占 81 40 % ,对照组 15眼占 36 5 9% ,2组比较(χ2 检验 ,P <0 0 5 )。结论 巩膜阶梯状切口 ,对于减少术后角膜散光 。
Objective To evaluate the effect of the corneal astigmatism from the scleral stepladder incision. Methods The scleral stepladder incision was performed in extracapsular cataract extraction and IOL implanatation on 43 eyes. The changes of corneal astigmatism was examined with the keratometer and optometry in early stage after operative and compaired with the control group of routine corneoscleral incision. Results Post operatively the average coneal astigmatism of the scleral stepladder incision group were (3.02±1.62)D at three days and (1.21±0.98)D at three months which were respectively less than (4.78± 2.21 )D and (2.90±1.36)D in the control group ( t test, P <0 05). The visual acuities without corretion were 0.5 or better on 35 eyes in 81.40% at one week which were better than 15 eyes in 36.59% the control group (χ 2 test, P <0.05). Conclusions These results suggest that following the scleral stepladder incision extra capsular cataract extraction and IOL implanation were better than routine corneoscleral incision on reducing postoperative corneal astigmatism and raising vision.
