胃癌及邻近黏膜表达胃泌素受体的差异及其意义

The difference of gastrin receptor between gastric cancer and neighbouring mucosa and its significance

目的揭示胃泌素受体（即ｇａｓｔｒｉｎｒｅｃｅｐｔｏｒ，ＧＲ）在胃癌自分泌生长中的作用。方法应用受体的放射配基结合分析法，测定３４例胃癌及其邻近黏膜的ＧＲ含量和亲和力（Ｋｄ），比较胃癌及其邻近黏膜表达ＧＲ的差异。结果３４例胃癌组织中，ＧＲ阳性者１６例。胃体癌表达高亲和力ＧＲ的阳性率为７７．８％（７／９），胃底贲门癌为５０．０％（３／６），而胃窦癌为２１．１％（４／１９）。晚期胃癌（Ⅲ、Ⅳ期）表达高亲和力ＧＲ的阳性率为５２.２％（１３／２４），早、中期胃癌（Ⅰ、Ⅱ期）为１０．０％（１／１０）。３４例黏膜组织中，ＧＲ阳性者１６例。有３０例胃癌及其邻近黏膜组织表达ＧＲ基本一致。胃癌表达高含量ＧＲ的平均含量较邻近黏膜高。结论 胃癌较周围黏膜易于表达高含量ＧＲ，其表达与肿瘤的部位和分期相关。

Objective To demonstrate the role of gastrin receptor (CR) in autocrine growth of gastric cancer. Methods Radioligand binding assay of receptor was used to determine gastrin receptor in specimens of gastric cancer and its neighbouring mucosa from 34 patients. Results 16 cases of gastric cancer had positive CR. High-affinity CR expression was present in 77. 8% (7/9) and 50.0% (3/6) in cancer of gastric body and fundus respectively, but only 21. 1% (4/19) in cancer of gastric antrum. The positive rate of high-affinity CR expression in Ill + IV stages (52. 2% ) was higher than that in I + II stages (10.0% ). 16 of 34 cases of gastric mucosa had positive CR. The expression of CR in gastric cancer was consistent with that in neighbouring mucosa in 30 patients. The average GR contents was higher in gastric cancer than that in neighbouring mucosa. Conclusions High-contents GR (CR > 10 final/mg. pro) is easier to be expressed in gastric cancer than that in gastric mucosa. The CR expression is correlated with the site and the stage of gastric cancer.
