慢性活动性乙型肝炎患者T细胞亚群状态的研究

T CELL SUBSETS IN CHRONIC ACTIVE HEPATITIS-B

应用单克隆抗体和ABC免疫酶标方法对30例慢性活动性乙型肝炎患者外周血及部分患者肝活检组织中T细胞亚群的分布和变化与健康对照外周血T细胞亚群比较进行了观察。结果表明:T_1阳性细胞比例无显著差异。T_4阳性细胞比例明显减少,T_8阳性细胞比例明显增加,T_4/T_8比值明显降低。P值均小于0.01。CAH-B患者肝活检组织中,肝细胞点、片状坏死区周围浸润的细胞为T_8阳性的淋巴T细胞。

The changes and distribution of T cell subsets in peripheral blood andpiecmeal necrosis of liver cells were defined by monoclonal antibodies and ABCtechnique in 30 patients with CAH-B. In comparison with healthy control,the percentage of T_4 cell in the patients decreased significantly (P<0.01), thepercentage of T_3 cell increased significantly(P<0. 01 ) .Therefore,T_4 /T_8 ratio was obviously lower (P<0.01). The greatest changes were found in severe cases ofCAHB, but there were no differences found in the patients during their convalesc-ence when compared with healthy control (P>0.2). Only T_8 positive cells werefound around the piecmeal necrcsis in biopsies of severe patients, the cellsmight be T_8 positive cytotoxic T lymphocytes associated with immunopathogenesis of piecmeal necrosis of CAH-B.
