系统型结节性脂膜炎骨髓受累分析

Analysis of bone marrow involvement status of systemic nodular panniculitis.

目的 探讨系统型结节性脂膜炎患者骨髓受累情况。方法 对 13例住院患者同时进行血象和骨髓象的检查与分析。结果 ①血象中红细胞减少 12例 ;白细胞减少 10例 ,其中中性粒细胞降低伴淋巴细胞增高 7例 ;3例白细胞总数增多。②骨髓象中骨髓增生活跃 12例 ,减低 1例 ;粒系增生减低 9例 ,活跃 3例 ;红系增生减低 3例 ,活跃 3例 ;淋巴系增生活跃 2例 ,余正常。结论 ①患者的血象异常来源于其骨髓受累 ,异常表现为红系和粒系有不同程度受抑、淋巴系受累较轻 ;②骨髓受累程度与病情轻重相关 ,轻症患者骨髓受损较轻 ,积极治疗可阻止病情发展 ;

Objective To investigate the bone marrow involvement status of systemic nodular panniculitis. Methods The myelograms and blood pictures were examined at the same time in 13 admitted patients with systemic nodular panniculitis. Results Among the 13 patients with systemic nodular panniculitis, erythrocytopenia occurred in 12 cases, leucocytopenia in 10(7 accompanied by lymphocytosis and neutoophilopenia), leukocytosis in 3 cases. Hypocellularity was verified in one case of 13 patients' bone marrow and others showed normal cellularity. But in the granulocytic series 9 cases showed hypocellularity and 3 cases hypercellularity. In the erythroid series hypocellularity and hypercellularity occurred in 3 cases, respectively. In the lymphocytic series all of the patients were normal except of 2 patients with hypercellularity. Conclusion The abnormal blood picture of the patients was contributed to the involvement of bone marrow. The granulocytic series and erythrocytic series were arrested to some extent in the bone marrow, but the lymphocytic series were little involved. Mildly affected patients had slightly involved myelogram and could be reversed by prompt treatment, but in seriously affected patients, it is difficult to reverse their involvement.
