雷公藤合并皮质激素在治疗天疱疮、类天疱疮中的作用

The effect of Tripterygium Wilfordii combined with corticosteroid in the treatment of pemphigus and pemphigoid.

目的 探讨雷公藤合并皮质激素在治疗天疱疮和类天疱疮中的作用。方法 在皮质激素剂量不足或高剂量下病情波动时以及皮质激素减量时合用雷公藤治疗 75例天疱疮和 2 5例类天疱疮。从雷公藤起效时间、皮损控制时间、激素抽减时间三项指标观察雷公藤对不同病情和类型的天疱疮、类天疱疮的作用 ,并与纯皮质激素组进行比较。结果 合用雷公藤无论对轻、中症或重症、病情稳定或波动的天疱疮和类天疱疮均有相似的疗效 ,皮损控制时间与激素抽减时间与纯皮质激素组无显著差异 ,各型天疱疮的总有效率为 74.7% ,类天疱疮为 6 8.0 %。结论 雷公藤在合并皮质激素治疗天疱疮。

Objective To study the effect of Tripterygium Wilfordii combined with corticosteroid in the treatment of pemphigus and pemphigoid. Methods Tripterygium Wilfordii was added in the treatment of 75 cases of pemphigus and 25 cases of pemphigoid under following circumstances: 1) state of the illness was fluctuating with high dosage of corticosteroid; 2) the dosage of corticosteroid was inadequate; 3) at the duration of reducing the dosage of corticosteroid. The effects of Tripterygium Wilfordii on diffe rent types and severity of pemphigus or pemphigoid were compared. The effects of Tripterygium Wilfordii with pure corticosteroid therapy from three aspects were compared also: the onset of the effect of Tripterygium Wilfordii, the duration which Tripterygium Wilfordii took to control the eruption and the duration that the dosage of corticosteroid could be reduced. Results Tripterygium Wilfordii and corticosteroid combined therapy had definite effect on pemphigus and pemphigoid with different severity, there was no difference between the duration that the combined therapy and pure corticosteroid therapy took to control the eruption and to reduce the dosage of corticosteroid. The total effective rate for pemphigus was 74.4% and that for pemphigoid was 68.0% . Conclusion Tripterygium Wilfordii could partly substitute for or reduce the dosage of corticosteroid in the treatment of pemphigus and pemphigoid.
