摘要
①目的 了解肥厚性心肌病病人冠状动脉特征。②方法 对２８例肥厚性心肌病病人行冠状动脉造影检查，观察其冠状动脉分布及特点，测量各分支中段内径大小，并与同期行冠状动脉造影检查的正常人进行比较。③结果肥厚性心肌病病人冠状动脉供血呈左优势者占２１．４％，明显高于正常人（Ｘ２＝１１．５７．Ｐ＜０．０１），各亚型间无明显差异。发现并发冠心病者５例，其中１例行经皮腔内冠状动脉成形术（ＰＴＣＡ）及冠状动脉内支架术，另１例植入永久性ＤＤＤ起搏器治疗；１例左前降支中段发现冠状动脉肌桥。无冠状动脉病变的２２例病人左主干、回旋支及右冠状动脉中段内径较正常人明显粗大（ｔ＝４．７３９～８．２６０，Ｐ＜０．００５）。④结论肥厚性心肌病病人冠状动脉供血呈左优势者比例较高，其冠状动脉内径较正常人粗大；疑有并发冠心病者须行冠状动脉造影检查，对指导治疗有一定意义。
Objective To observe the characteristics of coronary arteries in patients with hypertrophic myocardiopathy. Methods Coronary angiograms in 28 patients with hypertrophic myocardiopathy were analyzed for determining the distributions and characteristics of coronary arteries. The mid-segmental inner diameters of the left main(LM), left anterior descendent(LAD). left circumflex(LCX) and right coronary arteries(RCA) in the patients were measured respectively, and compared with those in the subjects without coronary lesions receiving coronary angiography in the same period. Results The left coronary predominance of myocardial blood supply was 21. 4 % in patients with hypertrophic myocardiopathy and there was no significant difference among 3 subgroups. 5 patients were found accompanying with coronary heart diseases, among them, one patient was implanted a DDD pacemaker and another one was treated with balloon PTCA, and a 15. 0mm ×3. 0mm stent placed in the posterior descending branch of the tight coronary artery. I patient was found a coronary muscle bridge in mid-segment of LAD. The inner diameters of LM, LCX and RCA in patients without coronary lesions were significantly more increased than those in the healthy subjects. Conclusion Our results indicate that the left predominance rate of coronary blood supply in patients with hypertrophic myocardiopathy was higher, and inner diameters of the coronary arteries were relatively larger. Coronary angiography must be performed for patients suspected with coronary heart disease, which is meaningful for diagnosis and treatment.
