摘要
为了探讨新生儿肾小管功能变化特点及测定尿视黄醇结合蛋白(RBP)的正常值,本文对1—10天内4个日龄组的114例正常新生儿进行了尿RBP、尿Cr测定并计算尿RBP/Cr比值。结果显示随着日龄的增加尿RBP有逐渐下降的趋势(~1d0.042±0.018mg/L、~3d0.035±0.019mg/L、~7d0.038±0.026/L),7天后明显下降(7~10d0.026±0.022mg/L)。说明7天后新生儿肾小管重吸收功能明显完善。本研究测得的尿RBP95%可信限的正常范围是:7天内小于0.0811mg/L、7天后小于0.0699mg/L。尿RBP/Cr比值在~7天组形成一个峰值,7天后也显著下降,与尿RBP相比尿RBP/Cr比值更灵敏地反映了肾小管功能在7天左右的变化。本文为窒息时肾小管损害提供了一个更灵敏的监测指标。
The purpose of the article was to search for the change of uriniferous tubules's function in normal neonatal infants and to detected the normal value of URBP. URBP. Ucr and URBP/Ucr were detected in 114 cases normal neonatal infants who was diveded into four groupes by ELISA method (two steps). The results showed that URBP value had declining trend with increasing of age. (<24h 0.0419±0. 0157mg/L, 1-3d 0.0354±0. 0192mg/L, 3-7d 0.0380 ± 0.0259mg/L). After 7 days it became obvious descent (>7d0.0257±0.0221mg/L). It showed that uriniferous tubules's reresorption function had been markedly improved after 7 days. In our results 95 percent confidence of URBP normal value was less than 0. 081mg/L within 7 days and it was less than 0.070mg/L after 7 days. There was a peak of URBP/Ucr vatio at 3rd days to 7th days. So that URBP/Ucr vatio sensitively reflected the uriniferous tubulis's function. This results proved a senstive index for clinical use to detect uriniferous tubules' function of asphyxia infants
