高度近视患者的视野改变与屈光性角膜手术

Changes of visual field and keratorefractive sur-gery in high myopia

目的 探讨高度近视患者的视野改变及屈光性角膜手术的效果。方法 35例患者 (70眼 )按屈光度分为 2组 : 组为高度近视组 ,屈光度 - 8.0 0～ - 10 .0 0 D,34眼 ; 组为超高度近视组 ,屈光度 >- 10 .0 0 D,36眼。常规眼前节及后节检查。应用 Octopus(瑞士 )视野计进行视野检查并分析。视野正常者可行屈光性角膜手术。结果 70例患者中 15眼 (2 1.43% )视野正常 , 组 (10眼 )高于 组 (5眼 )。 5 5眼 (78.5 7% )显示不同程度的缺损 , 组 (2 4眼 )明显高于 组 (31眼 ) (P<0 .0 5 ) ;≤ 30 a视野缺损比例低于 >30 a(P <0 .0 5 ) ;男女间无显著差异。视野缺损与视网膜退行性变化、近视度数、年龄等因素有关 ,其缺损形状无典型的特征性改变 ,颞上方较多见。 2 4眼行 PRK或 L ASIK,3mo、6 mo时实际矫正度在预期矫正度± 1.0 D范围内分别为 92 .4%、96 .2 % .结论 高度近视常伴视野缺损。因此 ,对高度近视眼患者进行视野检查 。
