包虫病人唾液特异性抗体的检测研究与应用 被引量:5

DETECTION OF SPECIFIC ANTIBODY IN SALIVA FROM PATIENTS WITH HYDATID DISEASE AND ITS APPLICATION

摘要 目的 研究包虫病人唾液中特异性抗体的诊断方法和唾液抗体的代谢特点。方法 应用ELISA、IHA和LA法检测包虫病人和正常人唾液中的特异性抗体。以ELISA法测定唾液抗体含量与取样时间的关系。进行包虫病唾液抗体的流行病学普查验证。结果 包虫病人唾液抗体IgG -ELISA、LA和IHA试验阳性率分别为 84.4% (119/ 141)、48.94% (2 3/47)、8.75 % (7/ 80 )。包虫病人唾液中特异性IgG抗体检出率明显高于IgA(P <0 .0 0 1)。对 141例包虫病人唾液和血清进行IgG -ELISA平行检测 ,血清IgG抗体阳性率 90 .0 7% ,总符合率为 85 .82 %。对 30例包虫病人于 2 4h内 5次不同时间进行唾液采样测定IgG抗体 ,发现IgGOD均值浓度差别不明显。包虫病唾液抗体普查阳性率为 16 .8% ,与影像学检查对比符合率为 6 1.9% (13/ 2 1)。结论 包虫病人唾液抗体ELISA检测法 ,操作简便、经济、灵敏度好 ,经过标准化后适合作为包虫病临床和流行病普查的免疫学诊断方法。 Aim In order to study the diagnosis method of specific antibody in saliva from patients with hydatid disease and the metabolic characteristic of saliva antibody.Methods The specific antibody in saliva from the patients and normal people were detected with the methods of ELISA,IHA and LA.ELISA was used to determine the relationship between the contents of saliva in antibody and the sampling time,and to verify epidemiological mass survey of the saliva antibody of hydatid disease.Results The positive rates of the saliva antibody of the patients detected by IgG-ELISA,LA and IHA were 84.4% (119/141),48.94% (23/47) and 8.75% (7/80) respectively.The detectable rates of specific IgG in the patients saliva were obvious higher than that of IgA (P<0.001).The parallel detection of IgG-ELISA of the saliva and sera of 141 patients with hydatidosis showed that the positive rate of 90.07% of IgG in sera was higher than that in saliva.There were 6 samples of the antibody positive in saliva but the negative in sera,14 samples of the positive in sera but the negative in saliva,113 samples of the both positive and 8 samples of the both negative,with the total coincidence rate of 85.82%.Sampling saliva from the 30 patients at five different times during 24 hours,IgG antibodies were detected for OD values.It indicated that during 24 hours,the mean of OD values of IgG was at the ascending period during 8-11 hours,then descented in the period of 11-20 hours and moved upward again during 20-23 hours.The screening positive rate of saliva antibody of echinococcosis was 16.8% and compared with the image examination,the coincidence rate was 61.9% (13/21).Conclusion The ELISA detection method of saliva antibody from patients with hydatid disease was excellent for its simple performance,economy and high sensitivity.After standardization,it is fit for immunological diagnosis method of clinic and epidemiological mass survey of hydatid disesse.
